BackgroundHip joint injury is a disease with a wide distribution, many factors, more complications, more serious harm and high cost of treatment, which makes the prevalence continue to rise in different countries. Patients suffering from hip joint fracture get a mortality rate as high as 27% and a disability rate up to 50% within one year. At present, domestic and foreign scholars on the cause of the hip joint injury hypothesis mainly focused on demographic, complications, habits, nutritional status and medical care and so on. The high death rate of the hip joint injury is mainly related to increasing age, using residential care, delirium, pulmonary embolism, cardiovascular complications, malnutrition and antibiotics, which can affect the rehabilitation. Patients with hip joint injury prone to attack repeatedly and cause two damage easily, furthermore, it has a high incidence of complications. Postoperative care plays an important role in the recovery of hip joint injury, which can effectively reduce the incidence of complications and improve the quality of patients’ life. Although the current attention to the hip joint injury patients are getting higher and higher around domestic or international, but the effect is not expected to reach the desired state, and fail to solve the problem of high cost of care yet. Quality control circle intervention has been widely used in the field of nursing, and has achieved good results in the domestic nursing work. However, the comprehensive evaluation of the application effect of quality control circle intervention on the rehabilitation after hip joint injury was not found at home and abroad. Therefore, this study mainly evaluates QCC in hip joint arthroplasty compliance and functional recovery, so as to explore a new method for hip joint injury nursing in future.ObjectivesLearn distribution hip joint injury patients, evaluate QCC rehabilitation of patients after hip injury, meanwhile, to explore an effective method that is more suited to hip joint injury rehabilitation nursing after hip surgery.Materials and MethodsThis study used a randomized controlled trial design, which randomly selected 100 cases of hip joint surgery patients as the intervention group in the Second Affiliated Hospital of Bengbu Medical College during 4-12 months in 2015, and the intervention factors are QCC activities. In accordance with the intervention group’s sex, age, culture degree, body mass index (BMI), disease severity and marital status matching principle, the control group marches 100 cases as contrast before QCC activities between 2014August and 2015March. According to the ten steps of QCC activities, the experimental group take functional exercise, while the control group product no QCC management. Data processing used Excel software. At different time points, Harris hip score compared with repeated measures design variance analysis, and, the two groups after surgery compared with functional exercise compliance rate and evaluation of QCC management after nursing intervention effect by chi square test, and statistical treatment of data used the software SPSS22.0.Results1. After carrying out the management, the compliance rate of functional exercise after hip surgery was increased from 48% to 80% after the development, and the improvement range was 60.44%.2. After repeated measures design analysis, it was found that the Harris hip joint score in the control group and the intervention group were different, and, with the increase of time, the score of the intervention group increased significantly higher than that of the control group.3. The control group and the intervention group were different between the pain relief score, and the intervention group was significantly higher than the control group, and the intervention group were gradually increased after 2 weeks,1 months and 2 months.4. The score of hip joint injury in the intervention group was significantly higher than that in the control group, and the difference was not obvious after 2 weeks, but increased significantly after one month.5. The difference between the two groups was not significant.6. The activity degree of the joint increased significantly after 2 weeks, the score increased significantly after 2 months, while the control group and the intervention had no significant difference at different time.7. Compared with the control group, the postoperative complications were significantly decreased in QCC activity, and the distribution of pressure ulcer, deep venous thrombosis and pulmonary infection were different in the two groups.Conclusions1. It is a long-term process about recovery after operation on the hip joint injury, which needs to improve the effect of treatment, nursing and rehabilitation in elderly patients continuously, to reduce the mortality and readmission rate, to recover the function of independence, to enhance the ability of activities of daily living, so as to improve the quality of life.2. QCC activity can effectively improve the compliance of patients with hip joint injuries, especially in the functional exercise compliance. The postoperative Harris score increased more obviously by QCC, especially in pain relief and functional level standards.3. QCC activity can effectively reduce the incidence of complications of the hip joint injury, which also has a very good auxiliary effect to the operation treatment.4.QCC activity can increase the communication between medical staff, enhance the cohesion of the medical staff, improve the details of the intervention of health care and communication.
