Objective:To provide basic data for neurosurgery in this region, improve the safety of the operation, reduce or even avoid severe complications by researching the exposure of anterior clinoid process (ACP), clinoid space and the surrounding structures and its relationships.Methods:10 dry skull samples of Chinese adults were studied. The size,distance and angle of the anterior clinoid process and its surrounding structures were measured.5 Chinese adult cadaveric heads were fixed to the simple head frame for anatomizing,and were dissected layer by layer by simulating the frontotemporal orbitozygomatic approach, and some important anatomical structures were observed,accurately measured and photographed under the microscope.All measured data were managed statistically with statistical software SPSS 17.0,and expressed with mean±standard deviation.Results:There were 3 cases (6 sides) that had bony bridge formations between ACP and media clinoid process,and 3 cases (5sides) between ACP and posterior clinoid process.The total length of ACP was 9.80±1.18mm, and the basilar width and the sickness of it was 12.51±1.48mm,5.94±1.44mm, respectively.The length and width of the optic strut was 5.23±0.96mm, 2.60±0.65 mm,respectively. The vertical distance from the medial border of optic canal (OC) to the lateral border of ACP was 10.59±1.50mm, and from the lateral border of OC to the lateral border of ACP was 6.02±0.58mm. The distance between the tip of anterior clinoid process and the most lateral margin of superior orbital fissure was 21.74±1.89mm. The distance between the lateral edge of ACP and the center of OC was 8.76±0.73mm. The angle between the lateral margin of ACP and the sagittal axis was 39.95±6.07°.The angle between OC and the sagittal axis was 39.60±6.11°.The clinoid space was the space that was formed artificially after anterior clinoid process was dissected,the tip of which pointed to the posteromedial direction.The basilar width and depth of clinoid space was 5.10±1.15mm,7.84±1.55mm respectively;the length of the internal,external,inferior side of the clinoid space was 7.98±1.62mm, 10.61±1.41mm,10.43±1.31mm, respectively; the width of its top edge was 2.34±0.89mm. The clinoid segment of the internal carotid artery was located between the distal and proximal dural ring and wedge-like,with the length of 5.01±0.49mm. The diameter of the ophthalmic artery was (1.61±0.59) mm.Conclusions:The operating space of ACP and its surrounding area was small and was surrounded by many important vessels and nerves.The extent of anterior clinoidectomy be limited within the Dolenc triangle. The clinoid space was formed following the anterior clinoidectomy,which expanded the operation space, increased the exposure of the optic nerve, the ophthalmic artery and the internal carotid artery.
