Objective:To understand the status of medical practice environment, and analysis the status of medical practice environment about working environ-ment,personal values reflection,career satisfaction,social environment,system environment,etc.Exploring the influence factors of current practice environment present situation,aiming at the existing problem of medical practice environ-ment,and putting forward some corresponding suggestions and countermea-sures,and providing some thoughts and basis for improving the medical practice environment present situation.Methods:1000 subjects who from eight different grade hospital in Nanning were randomly selected as respondents for randomly stratified sampling invest-tigation.Using statistical analysis methods to describe amd analysis the current situation of medical practice environment.Variance analysis and correlation analysis was carried out on the influence factors.Results:(1)Working Environment:Working strength aspects:84.7% of res-pondents working hours overtime every week.In good health subjects with 35.8%.Professional risks, evaluation of operation risk big respondents with 32.1%,72% of respondents experienced medical violence.Working boredom with 32.9% of those polled in a recent years had to quit.(2) Personal Values Reflection:Treatment of identity,53.5% of respondents are not satisfied with the current of the relationship between income and pay,61.4% of respondents felt the income below their labor pay.Practice self-discipline,when hospitals need to pay for themselves,34.5% of respondents are always for the patient,29.6% of respondents will start from the patient’s actual condition.(3)Career Satisfacti-on:Practice will feel respect,40.9% of respondents were willing to once again choose to work in the medical,57.4% of respondents don’t want to their children in the medical work,43.5% of respondents think the work can give full play to their talents.Social status identity,32.9% of respondents of relatives and friends support their career,47.8% of respondents think media reported being short of justice and objectivity,29.3% of respondents felt their social status is lower than the general population.(4)Social Environment(the doctor-patient relationship): 24.5% of respondents believe patients trust them,23.2% of respondents believe patients respect them,28.3% of respondents were violated by patients at work. (5)Policy System Environment:33.5% of respondents thought to deepen medi-cal reform policies to improve the practice environment,40.6% of respondents believe that grading diagnosis and treatment policies to improve the medical care problem should continue to promote,32.7% of respondents believe the current two-way referral system work well. Hospital culture,43.4% of respond-ents thought there is a good "inheritance, help and missionary" atmosphere in their hospitals,37.5%of respondents thought title promotion is fair in their hospital,41.5% of respondents their hospital attaches great importance to the worker cultural quality and moral cultivation,34.4% of respondents thought their hospitals provide convenience for them to continue education. Crisis manage-ment in the hospital, the hospitals which the 41.2% of respondents working for able to "provide for the physical security", the hospitals where the 37.8% of respondents working "purchasing medical liability insurance for the employees", the hospitals that 32.9% of respondents working for can "provide legal support.", the hospitals where 21.6% of respondents working with a department respon-sible for handling medical disputes ",16.7% of respondents hospital where there is" providing mental health counseling".Conclusions:(1)Long Working Hours,work intensity,poor health conditions. Medical workers overtime general, the higher the proportion of hospital medical staff the higher the level of overtime.Weekly overtime work, mostly doctors, nurses,medical technicians, managers work less overtime, the overtime work the crowd,mostly young medical workers,long working hours affect the physical health of medical workers.(2)Practicing High-risk, the risk of a wide range of sources,including not only the risk of iatrogenic practice environment chara-cteristics brought, as well as from medical disputes, such as practicing risk of complaints, lawsuits brought by medical violence, as well as medical unknown, self-knowledge limitations practice risks.(3)Factors for Separation Intention produced more easily work tired. Turnover Intention of factors that include their own development prospects are confused,too busy to take care of the family feel remorse,negative psychological changes occur, work against their will and so on. Working hours,physical health,practicing risk and turnover intentions were related.(4)Personal Values Reflect Aspects:income and does not match the pay, benefits low sense of identity practicing self-discipline high.(5)Career Satisfac-tion:the practice is not strong,low social identity(6)Social Environment:Diff- erential Analysis:survey different titles in the doctor-patient relationship of trust difference, the higher the title, the more I trusted patient, different levels of hospitals surveyed have differences in the relationship of trust between doctors and patients, the higher the hospital level, the more I trusted patient. Correlation Analysis:The degree of confidence in the past year, whether the patient had been suffering from side violated (abusive, threatening, personal attacks, etc.) may have relationships (P< 0.05, R=-0.274), the higher the level of trust of their patients, in the past a year been subjected to fewer violations of suffering side, and the degree of respect for the patient over the past year is subject to the risk of side violated (abusive, threatening, personal attacks, etc.) may have relationships (P<0.05, R=-0.118),patients the higher the degree of respect in the past year been subjected to violations of the less risk of side.(7)System Environment:good policies and systems,job promotion fairness needs to be strengthened,hospitals and cultural systems need further improvement,hospital crisis management measures should be more comprehensive.
