| Purpose:The aim of the present study was to investigate the difference in the liver tolerance, the size of spleen and the blood platelet in colorectal cancer patients after oxaliplatin based chemotherapy with or without bevacizumab.Methods:From April 2013 to December 2014,93 adult patients with colorectal cancer and liver metastasis were evaluated retrospectively. The 93 cases were treated with bevacizumab combined with oxaliplatin-basde chemotherapy (bevacizumab(+)group) and oxaliplatin-basde chemotherapy alone (bevacizumab(-)group). Patients were assessed the liver function, albumin, platelet, the size of spleen before and after the cancer treatment.All patients were also followed up to observe the 1 year progression-free time and 1 year survival rate.Results:Before the first cycle of chemotherapy, the basic albumin of colorectal cancer patients treated by bevacizumab plus oxaliplatin-basde chemotherapy have no significant difference in the group bevacizumab(-)(42.51g/L vs 41.43g/L,P=0.517). After the chemotherapy and chemotherapy combined with bevacizumab, the decrease of albumin occurred more in bevacizumab(+) group(30.19% vs 52.50%, P=0.030). There was no significant difference in the increase of ALT and AST between the two groups after the chemotherapy treatment.There was a significant difference in the incidence of thrombocytopenia between the two groups,75.47% vs 55.00%, P=0.038.The time of initial thrombocytopenia in colorectal cancer patients of bevacizumab (-) group was significantly 1.5 cycle earlier than the bevacizumab (+) group (P=0.013). There was also difference in splenomegaly between the two groups, and it happened earlier in bevacizumab (-) group than bevacizumab (+) group (5.71 cycle vs 7.14 cycle,P=0.030). Thrombocytopenia was found more in patients with splenomegaly,27/29 vs 35/64 (x2=13.254, P<0.01). The survival rate in 1 year was 81.13% in bevacizumab (-) group and 85.00% in bevacizumab (+) group, while the recurrence rate was 22.63% and 15%. The TTS of bevacizumab (+) group was significantly longer than bevacizumab (-) group (T=-1.956, P=0.047).Conclusion:Colorectal cancer patients’ liver transaminase increased after the treatment with bevacizumab combined with oxaliplatin-based chemotherapy or oxaliplatin-based chemotherapy alone.Thrombocytopenia and splenomegaly were found more frequent in bevacizumab (-) group. This reduction of platelet and spleen enlargement maybe have something with the injury of liver sinusoid caused by the chemotherapy. |
