The Clinical Observation On Preventing The Recurrence Of Non Muscle Invasive Bladder Cancer After Operation And Reducing The Side Effects Of Intravasical Instillation By Xiaozheng Decoction

Research Background:The bladder cancer isone of the most common tumor of urinary system in our country. The incidence of bladder cancer in all malignant tumors ranked seventhin the male rate,and ranked after tenth in female rate. At the same time, the mortality rate is 3.75/100,000and 1.24/100,0000 respectively. The transitional cell cancer is the main tissue type which accounted for more than 90%.Non muscle-invasive bladder cancer (NMIBC) accounted for 75-85% in all bladder cancer. Transurethral resection of bladder tumor (TUR-BT) is the golden standard for NMIBC.10% to 67% patients after TUR-BT will have the bladder cancer recurrence within 12 months. How to reduce the rate of recurrence after transurethral resection of bladder cancer is an important problem.Bladder irrigation chemotherapy drugs are the most common used method for preventing post-TUR-BT recurrence.The commonly used drugs include Pirarubicin,Hydroxycamptothecin, Mitomycinis andBCGin the domestic. However the recurrence rate is not satisfactory. At the same time,the immune preparation or chemical drugs of any bladder perfusion will lead adversereaction of frequent micturition,urgency,pelvic pain, hematuria, fever, loss of appetite, or contracture of bladder, bone marrow suppression in severe cases, that patients therefore had to give up the perfusion of bladder.Traditional Chinese medicine has not onlyimmunomodulatory effects and anti-tumor effect, but also can reduce the perfusion of bladder irritation symptoms,by which to help fulfill perfusion therapy.Thus, traditional Chinese medicine will have a brilliant future in bladder cancer treatment by exert its advantages.The doctors of Department of Urology,Guang’anmen hospital,had a wealthy experience in preventing recurrencerate and reducing the side effects of instillation of bladder of non muscle invasive bladder cancer after transurethral resection by using Xiaozheng decoction which was created by Professor Liu Youfang. Professor Zhang Yaqiang, who has learned from Professor Liu Youfang for long years thinks that the main pathogenesis of bladder carcinoma after operation is deficiency in origin and excess in superficiality. The former refers to the deficiency of vital qi, especially spleen and kidney qi,while the latter refers to damp,toxin and blood stasis.By using Xiaozheng decoction,Professor Zhang Yaqiang had received the effect of reducing the postoperative recurrence rate of bladder tumor and the side effect of perfusion. Object:To observe the effect of recurrence and effect of intravesical instillation of side effect of transurethral resection of bladder cancerafter operation by Xiaozheng decoction.Methods of treatment:Do bladder perfusion with Pirarubicin on 88 patients with non muscle invasive bladder cancer patients who have had TUR-BT.Treatment course:once per week,8 times in total, later once per month 10 times in total.At the same time, every patient takes Xiaozheng decoction until 2 years after operation.Data analysis:1. To count the 2 year recurrence rate, and document compare were applied.2.To count theincident rate of irrigation of bladder,and document compare were applied.Result:1.The 2 year recurrence rate were better than literature reportage thatbladder perfusion with Pirarubicin only.2.The incident rate was better than that perfusion with Pirarubicin only in the literature.
