Objective:Specific serum protein biomarker was screened and identified in patients with relapse/refractory diffuse large B cell lymphoma to improve diagnosis and treatment. Methods:62 cases patients with newly diagnosed diffuse large B cell lymphoma were screened by Surface Enhanced Laser Desorption/Ionization Time-of-Flight Mass Spectrometry(SELDI-TOF-MS),a protein peak with M/Z 11—12kDa(lost goodwill target, LGT)was revealed in patients who were suffering from diseases progression,replased and dying, which was identified as serum amyloid A, and three cpmpanied protein peaks were also revealed :6.107 kDa,7.974 kDa and 8.067 kDa,which were positive correlated with LGT. On this basis, 86 fasting venous blood(3ml) were drawn from patients with newly diagnosed diffuse large B cell lymphoma admitted to Oncology Ⅲ Department in Shanxi Dayi Hospital from June,2014 to December,2015, centrifuged at speed 3500rpm(4℃) for 10 minutes and stored at-80℃. After receiving R-CHOP for 2-8 circulars, 30 relapse/refractory patients with integrated clinical data were involved in the study, another 30 patients in remission with same sex and similar age were taken as controls. 10 adenolymphitis patients and 10 healthy cases were negative controls. All serum samples were subjected to Tricine sodium dodecyl sulfate polyacrylamide gel electrophoresis to detect and compare the differentially expressed polypeptide. The differentially expressed polypeptides were identified by shotgun tandem mass spectrometry.The identified polypeptides were analyzed using bioinformatics.Dates were analyzed with SPSS18.0. Results:5 peptides band differentially expressed in the 30 relpase/refractory patients were identified by Tricine SDS-PAGE-as follows:(1)band a、b, molecular weight 10kDa-15 kDa,abnormally upregulated in 25(83%) patients suffering from relapse/refractory diffuse large B cell lymphoma patients’ serum electrophoregram and deleted in other three groups, chi-square test showed the differential expressed polypeptides between the relapse/refractory patients and patients in remission was of ststistical significance, it could serve as biomarker for relapse/refractory diffuse large B cell lymphoma.(2)band c,molecular weight 10 kDa,band d and e,molecular weight15kDa-20 kDa,downregulated or deteled in relapse/refractory patients’ serum electrophoregram, respectively, chi-square test showed the differential expressed polypeptides between the relapse/refractory patients and patients in remission was of ststistical significance,it could be used for predicting rapid progression and poor prognosis.2.5 differential peptides in relapse/refractory diffuse large B cell lymphoma patients were identified : a-serum amyloid protein A、b-degradated or misfolded transthyretin、c-apolipoproteinAⅡ、d-fragments of haptoglobin、e-natural transthyretin. Conclusion:5 peptides band differentially expressed in relapse/refractory patients were identified:SAA was identified previously and closely related to the relapse/refractory of malignant tumor.Degraded ormisfolded transthyretin and serum amyloid A were upregulated in relapse/refractory diffuse large B cell lymphoma patients’ serum, could serve as biomarker for relapse/refractory diffuse large B cell lymphoma.Apolipoprotein A-Ⅱ、undegraded or normal folded transthyretin、fragment of haptoglobin downregulated or deteled in relapse/refractory patients’ serum, also could be used for predicting rapid progression and poor prognosis.
