Clinical Analysis Of Related Factors Of Recurrence After VATS Of Primary Spontaneous Pneumothorax

Objective:Primary spontaneous pneumothorax( PSP) is the common thoracic surgery clinical acute disease.The method of treatment mainly has: The conservative treatment and the surgical treatment.The conservative treatment includes: simple observation, symptomatic and supportive treatment, thoracocentesis, thoracic drainage,chemical pleurodesis, and so on.The surgical operation treats include: traditional thoracotomy and VATS(Video-Assisted Thoracic Surgery).The goal in the management of PSP is to treat the acute episode,recover the pulmonary function and prevent recurrence. Its clinical therapeutic outcome is good, all can be cured and discharged. But it has the characteristic of repeated attacks,which is the clinical treatment difficult problem. After the conservative treatment has the high recurrence rate, the recurrence rate is between 16-52 %, equally 30 %, its recrudescence risk obvious enlarge along with the number of recrudescence increase. The surgery may remarkable reduce its recurrence rate, but the recurrence rate still had3.5-9.4 %, the average 5.5 %[1].Thus postoperative recrudescence is often a challenging clinical problem, at present factors associated with postoperative recrudescence analysis is the clinical research hot spot. The major aim of this study was to investigate factors associated with postoperative recrudescence, and optimizes the primary spontaneous pneumothorax surgical treatment, reduce its recrudescence, enhance the clinical curative effect and lighten patient’s economic burden.The method:Retrospectively analyse 169 patients with spontaneous pneumothorax underwent VATS treatment between January 2012 and December 2014 in shanxi provincial people’s hospital. We performed the statistical analysis by using SPSS18.0 software,we use chi-square test and t-test to evaluate the data.The result:Follow-up of 169 patients underwent VATS,10 cases postoperative recurrence of pneumothorax, the recurrence rate is 5.92 %.univariate analysis showed the primary spontaneous pneumothorax patient’s smoking, Body Weight Index, the postoperative pulmonary rehabilition time, the pleurodesis has the relations to the postoperative pneumothorax recrudescence(P< 0.05), while the gender, incidence of part, the age, the pneumothorax episodes times of preoperative has no relation to the pneumothorax recrudescence(P>0.05).conclusion:Primary spontaneous pneumothorax recurrence After VATS treatment has relationship with the smoke, the under-weight body weight index, the postoperative pulmonary rehabilition time are long, the pleurodesis, but has no relationship with gender,incidence of part, the age, the pneumothorax episodes times of preoperative.We thought regarding the smoking and the low body weight index’patients with primary pneumothorax should give the pleurodesis, lay the top and bottom each drainage tube postoperative,promotes the effect of thoracic drainage and early recove of lung tissue.After this may effective reduce this kind of patient recurrence After VATS treatment.
