Objective:Application 4-Dimensional Auto Left Ventricular quantification(4D Auto LVQ)evaluationon left ventricular systolic function in patients with uremia and patients with essential hypertension, and combining the four-dimensional Strain(4D) comparative analysis differences ofleft ventricular myocardial Strain in uremia patients and essential hypertension patients on the same level of blood pressure, provide a new method to evaluation cardiac function of both for clinical.Methods: Dividedpatients into three groups,69 cases of clinicallydiagnosed patients with uremia(uremia group)、39 cases of clinically diagnosedpatients with essential hypertension(hypertension group) and 45 cases of normalcontrols(control group).Application of GE Vivid E9,configuration 4V full volume and M5 S cardiac probe.To collect real-time three-dimensional echocardiography and import Echo-PAC workstation,start 4D Auto LVQ software, automatic measure LVEDV, LVESV, SV, CO, EF, EDMass, ESMass, etc.Finally start the 4 D Strain,get the golbal and 17 section strain values of left ventricular, and to express color coding "bull ’s-eye map".All data arecollected by using SPSS18.0 software for analysis.Results:1. LVEDV, LVESV, SV of uremia groupwere significantly increased than hypertension group and control group,difference have statistical significance;There was no statistically significant difference between hypertension group and control group;LVEDV of uremia group there were negative correlation with LVEF(r=-0.409,P<0.001).2.Uremia and hypertension group of LVM than control group were significantly increased, the difference was statistically significant; LVM of hypertension group increased than control group, the difference was statistically significant; LVM of uremia group there were negative correlationwith LVEF(EDMass:r=-0.891,P<0.001;ESMass:r=-0.887,P<0.001).3.Three groups of LVEF is measured with Simpson’s, the difference between three groups had no statistical significance;LVEF of uremia groupwere reduced than hypertension group and control group with4 D Atuo LVQ, the difference was statistically significant; There was no statistically significant difference between hypertension group and control group. 4. Uremia and hypertension groups ofGLS、GCS、GAS、GRS than control group were significantly reduced,difference have statistical significance;hypertension group than controlgroupalso reduce, the difference was statistically significant;GLS, GAS and LVEF exist positive correlation of uremia group(GLS:r=0.763,P<0.001;GAS:r=0.727,P<0.001).Conclusion:1. 4D Auto LVQcombine with 4D Strain can more quickly and accurately evaluation the change of left ventricular systolic functioninpatients with uremia and essential hypertension in the early.2.Under the same level of blood pressure,the myocardial damage in patients with uremia were moreserious than essential Hypertension.
