Abstract/Summary:
Purpose: as the complication of diabetic retinopathy, retinal vein obstruction, chronic uveitis disease, macular edema damages the eyesight, which may lead to irreversible blindness if not treated timely. Currently there is no ideal therapeutic agents for macular edema. Laser photocoagulation is still the preferred and basic treatment for macular edema. But during follow-up offen injuried after conventional Laser photocoagulation, and often appear paracentral scotoma,and increasing light curing scar, retinal color prime epithelial atrophy and so on. In the early 90’s, the concept of micro pulse laser in the treatment of macular disease was proposed. It can treat retinopathy through the use of short high frequency threshold value of therapeutic doses, and its advantages is to produce a retinal pigment epithelial spot, at the same time, to spread around the retina and choroid limitedly to a minimum degree, which could achieve therapeutic effect and reduce complications. The present study is to observe the changes of the best corrected visual acuity(BCVA), the macular central retinal thickness(CMT) and the complications before and after the treatment of subthreshold micropulse laser(577 nm) photocoagulation for macular edema.Methods:59 eyes of 40 patients who were confirmed as macular edema by fluorescein fundusangiography(FFA)and optical coherence tomography(OCT), and who had BCVA lower than 0.5, were included between June 2014 to June 2015 in ophthalmology department of the First Affiliated Hospital of Shihezi University.After 577 nm subliminal yellow micro pulse laser treatment,compare the change of BCVA and CMT Before and follow-up 1 week, 1month, 3 months, 6 months after the treatment. Data were input into SPSS 19.0 for statistical analysis and measurement data were expressed by x ±s.The mean and standard deviation of BCVA and CMT before treatment and at each follow—up after treatment were calculated. the BCVA and CMT were compared by paired t test before treatment and at each follow—up after treatment,and the difference of P<0.05 was statistically significant.Results : 59 eyes of 40 patients were involved in the treatment and follow-up after the treatment, in which no 1 case was lost to follow-up. after 577 nm threshold of the yellow laser photocoagulation treatment, examine the fundus, a significant macular laser scar was observed in no case, pigmentation with light coagulation fade gradually with the extension of treatment time. No postoperative complications such as neovascularization and fibrosis occurred. Mean corrected visual acuity test(CMT): the difference was not statistically significant(P > 0.05) before and follow-up 1 week, 1 month after the treatment. the difference was statistically significant(P < 0.05) before and follow-up 3 month,6 month after the treatment. average macular retinal thickness reduced significantly.Best corrected visual acuity(BCVA) testing: the difference was not statistically significant(P > 0.05) before and follow-up 1 week, 1 month,3 month after the treatment., the difference was statistically significant(P > 0.05) before and follow-up 6 month after the treatment. The average macular retinal thickness(CMT) changes earlier than the improvement of best corrected visual acuity(BCVA).Conclusion:1. Subthreshold micropulse laser(577 nm) photocoagulation can not only alleviate macular edema and enhance best corrected visual acuity,but also improve visual function.2. Subthreshold micropulse laser(577 nm) photocoagulation can effectively reduce the average macular retinal thickness(CMT) and alleviate macular edema.3. after Subthreshold micropulse laser(577 nm) photocoagulation, a significant macular laser scar in the fundus was not observed in any case, pigmentation with light coagulation fade gradually with the extension of treatment time. No postoperative complications such as neovascularization and fibrosis occurred during follow-up.
