Objective Analyzing the species and clinical features of dermatomyositis(DM) and polymyositis(PM) complicated with lung diseases in order to improve the knowledge of these diseases, so clinical doctors can early discovery, diagnosis and treat.Methods Totally 54 cases of hospitalized DM/PM patients with pulmonary disease, who are admitted to the First Affiliated Hospital of Chongqing Medical University from January 2011 to November 2014 as the objects.The general condition, clinical manifestation, laboratory examination, pulmonary function, imaging, treatment and prognosis were retrospectively analyzed. The objects of study according to the types of complicated with lung diseases can be divided into different groups.Results DM/PM complicated with lung diseases are interstitial lung disease(ILD), lung cancer and pneumomediastinum.DM/PM complicated with ILD occurs most frequently, whose respiratory common symptoms are cough, shortness of breath. The rate is 19.3%.Elevated erythrocyte sedimentation rate(ESR),C-reactive protein(CRP),creatine kinase(CK),ferritin and positive anti-Jo-1 antibody are often seen in DM/PM with ILD.The abnormal lung function mainly displays as diffused and restrictive ventilatory dysfunction. Ground glass is the most common sign in thoracic CT. Cutaneous vasculitis and malignant erythema are obvious in DM/PM complicated with lung cancer.80% patients have abnormally elevated CK and negative anti-Jo-1 antibody.DM/PM complicated with pneumomediastinum is a rare but serious complication.66.7% patients have significant skin vasculitis.All patients were young men, who have lower CK, increased ESR, negative anti-Jo-1 antibody.Conclusions 1 、 DM/PM patients complicated with lung disease are associated with corresponding inflammatory and immunological indexes.2、ILD is the most common pulmonary complication.But we should be aware of the other rare and serious complications.
