High intensity focused ultrasound(HIFU) has become a new method for clinical treatment of uterine leiomyomas for it’s security, effectiveness, less adverse effects, and the integrity of the organs. Studies have found that there was a great difference on the therapeutic effects of HIFU ablation uterine myomas, many leiomyomas are easy to ablate, but some are not, or cannot get the ideal effects even increas ing the dosage. While the mechanism of that difference have not clear, the histopathology components is an important factor which could infect the therapeutic effects of HIFU ablation. Thus, the study start with the histopathology components aspects, to study the correction between the histopathology components and the therapeutic effects of HIFU ablation, and explain the mechanism of the different therapeutic effects in HIFU ablation of uterine fibroids.Studies have found that the Magnetic resonance(MR)-T2 weighted imaging(T2WI) s ignal can be a forecaster for the therapeutic effects of HIFU, but HIFU ablation is a non-invasive treatment method, that cannot doing the histopathology examination. Thus, we designed to create the correction between the histopathology components and MR-T2 WI signal, to provide the basis of MR-T2 WI signal forecast the therapeutic effects of HIFU ablation of uterine fibroids.Objective(1) Quantitative detect the smooth muscle cell(SMC)、collagen fiber(CF) and MR-T2 WI signal in uterine fibroids, to explore the correction between the histopathology components and MR-T2 WI signal of uterine fibroids.(2) Explore the correction between the therapeutic effects of HIFU ablation the uterine fibroids specimens and components, explain the mechanism of the different therapeutic effects in HIFU ablation.Materials and methods(1) 46 uterine fibroids ablated by “hysterectomy” were collected from the First Affiliated Hospital of Chongqing Medical Univers ity from Dec 2014 to June 2015, all patients will do the routine MR scan before operation. Collecting the specimens to do the quantitatively analyze of the SMC、CF、 MR-T2 WI signal of uterine fibroids specimens after hysterectomy. And detecting the correlation coefficient between SMC number、CF contents and the scaled signal intensity of MR-T2 WI, to explore the correction between histopathology and MR-T2 WI signal in uterine fibroids, and the correction between pre-hysterectomy MR-T2 WI signal and post-hysterectomy MR-T2 WI signal.(2) HIFU ablate the specimens and measuring the energy effic iency factor(EEF). And detecting the correlation coefficient between EEF and the histopathology components of uterine fibroids.Results(1) 44 specimens diagnosed with uterine fibroids in the 46 tissue. The scaled signal intensity of MR-T2 WI in pre-hysterectomy is 0.86±0.24(0.54-1.43), the scaled signal intens ity 0f MR-T2 WI in uterine fibroids specimens is 0.73±0.20(0.41-1.25),correlation study have found that: there’re positive correction in the scaled signal intensity of MR-T2 WI between pre and post-hysterectomy.(2) the SMC number of the 44 specimens is 242.88±57.11(160.33-380)/vision, the CF contents of the uterine fibroids specimens is 50.05±14.11(24.21-83.10)%/vis ion. Correction analys is have found that it has a positive correction between SMC number and the scaled signal intens ity of MR-T2 WI in post-operation; There has a negative correction between CF contents and the scaled signal intens ity of MR-T2 WI in posthysterectomy.(3) The EEF of 44 specimens which have diagnosed with uterine fibroids is 7.04±1.81(3.66-10.36)J/mm3. Correction study have found that the correlation between the EEF of the specimens and the SMC number were positive correction; The correlation between the EEF of the specimens and the CF contents were negative correction.Conclusions(1) There is a correction between the histopathology components and the therapeutic effects of HIFU ablation of uterine fibroids, the difference in the histopathology components might be the reason for different therapeutic effects of HIFU ablation of uterine fibroids.(2) There is a correction between the histopathology components and MR-T2 WI signal, which provide the evidence of forecast the therapeutic effects of HIFU ablation of uterine fibroids by MR-T2 WI signal intensity.
