Purpose:Discussion prostate cancer(PCa) and benign prostatic hyperplasia(BPH) characterized in DTI imaging and DCE-MR imaging, and explore the clinical value of DTI, DCE-MRI, serum prostate-specific antigen(PSA) and combined DTI with DCE-MRI in the differential diagnosis of BPH and PCa.Materials and methods: Collected mainly with urinary frequency、 hematuria、dysuria and blood sperm and other urinary symptoms 43 patients for treatment. All patients underwent scanning with conventional MRI, DCE-MRI and DTI and improve the PSA-related inspection, get tracking biopsy and surgical pathology. Evaluated the MRI imaging features of lesions by high qualification of doctors then contrast with pathological. Selected and draught the region of interest(ROI) of DCE-MRI and DTI, measure and record the FA and ADC value of each group and the toping of SI-T curve. Analyze and record the relative peak signal strength and peak time information. FT’s three- dimensional reconstruction observed and recorded the fiber bundle continuity, out of shape and density conditions. Using statistical software for statistical analysis, calculation of the indicators of diagnostic performance in DCE-MRI and DTI for PCa and BPH; Assessment PSA and combine DTI with DCE-MRI the two ways of diagnosis performance of PCa.Results: 1, PSA accuracy、sensitivity、 specificity in the diagnosis of prostate cancer was 53.5%、95%、17.4%; 2, The ADC values of PCa group、 BPH group was 0.79 ± 0.30 × 10-3mm2/s, 1.32 ± 0.31 × 10-3mm2/s,(t=5.724, P=0.000); 3, The FA values of PCa group、BPH group were 0.49 ± 0.17、0.23 ± 0.10,(t=6.154, P=0.000); 4, The AUC of FA value is 0.901, the sensitivity、specificity、Youden index of is 0.91、0.82、0.73, the cutoff value for the diagnosis is FA = 0.19; 5,The fiber bundle of PCa cancer area traveling disorders, local sparse, interruption or disappeared; the outer periphery fiber bundle of BPH is significant downward pressure on thin and the local fiber bundles density increased, meanwhile the central fiber bundle thickening、local sparse; 6, PCa lesions showed a homogeneous enhancement, BPH lesions is significantly heterogeneous enhancement; PCa compared BPH with early time to peak; 7, PCa and BPH with differ SI-T curve types, BPH group were mianly with type Ⅰcurve meanwhile PCa group based on type Ⅲ curve; 8, Combine DCE-MRI and DTI accuracy of PCa diagnosis was 90.7%, sensitivity 90%,91.3% of specificity, Youden index was 0.813.Conclusions: PSA for prostate disease with high sensitivity but poor specificity; There are significant differences in BPH and PCa of DTI parameters, FA values have high diagnostic value for PCa, FT diagram can be visualized the fiber bundle change of BPH and PCa; PCa and BPH have differ in SI-T curve type, PCa`s peak time is shorter than BPH and PCa lesions showed a homogeneous enhancement; Combine DTI and DCE-MRI have high diagnostic value for PCa and BPH.
