Objective:The aim of this study was to explore the effect of pentazocine on the changes in cardiac troponin I and its association with calcitonin gene-related peptide in leg ischemia reperfusion induced by tourniquet,which used in orthopedic surgery. Methods:Sixty patients, ASA I or â…¡,undergoing elective unilateral leg surgery,which were carried out under spinal anesthesia, using tourniquet, were randomly assigned to control group(group C, n=30) and pentazocine treatment group(group P, n=30),in accordance with a random number table. Patients in group P pretreated with pentazocine(0.5mg/kg, in 100 ml of 0.9% saline, intravenously infused in 30 min) before surgery,while only the same volume of saline was infused intravenously in group C at the same schedule. Record the heart rate(HR),the saturation of pulse oxygen(SPO2) and mean arterial pressure(MAP) of patients at the moments of before inflating tourniquet(T0),before deflating tourniquet(T1),1minute(T2),30 minutes(T3) and 24 hours(T4) after tourniquet deflation. Record the time of duration of tourniquet, the volume of liquid infused, blood lost and urine generated in operation. 5 ml of blood samples were collected from cubital vein in an not transfusion arm immediately before inflating tourniquet(T0), 30 minutes(T3) and 24 hours(T4) after tourniquet deflation. Serum concentration of calcitonin gene-related peptide and cardiac troponin I were measured by using enzyme-linked immunosorbent assay(ELISA). Results:There were no statistical difference detected in the general status, time spent in surgery and patient status during operation in two groups. Compared to that at T0, the concentration of cTnI in group C was increased slightly at the time of T3(P>0.05), while increased significantly at the time of T4(P<0.05), the concentration of cTn I in group P were no obvious changes at the times of T3 and T4(P>0.05). The concentration of CGRP in group C were no obvious change at the time of T3 and T4(P>0.05),while CGRP in group P was increased significantly at the time of T3(P<0.05)and continue increased at the time of T4(P<0.05), compared to that at T0. Compared with group C, cTnI in group P was no difference observed at the time of T3(P>0.05),while decreased significantly at the time of T4(P<0.05). CGRP in group P was increased obviously at T3(P<0.05), while greater level of CGRP was detected at T4(P<0.05). Conclusion:The result of this study indicates that pentazocine may attenuate the increase of cardiac troponin I induced by leg ischemia reperfusion induced by tourniquet used in leg orthopedic surgery, in which endogenous CGRP may participate.
