Objective:To evaluate the value of three-dimensional saline-infused sonohysterography in the diagnosis of submucous myoma.Methods:42 female patients who were suspected submucous myoma from Jan,2013 to Dec,2015 in Shanxi Dayi Hospital were enrolled in this study. All patients had sexual life and their ages between 24-65 years old, with an average age of 44 years old. All patients were examined by transvaginal ultrasonography(TVS) and three-dimensional saline-infused sonohysterography(3D-SIS) in 3 to 7 days after menstruation was clean.Kept detailed records of whether there are intrauterine lesions and the information of pathological changes such as the type, location, quantity. After ultrasound examination,all patients were examined by hysteroscopy(HS), detailed record check information again and obtain the results of pathological examination. Using SPSS13.0 statistical software, by the pathology as the gold standard, Kappa test was used to the consistency of examination results of transvaginal ultrasonography, three-dimensional saline-infused sonohysterography, hysteroscopy and pathological results.Results:The five items(accuracy, sensitivity, specificity, positive predictive value, negative predictive value) of 3D-SIS in the diagnosis of submucous myoma were 90.5%,96.8%,72.7%,90.9%,88.9%. The five items of TVS in the diagnosis of submucousmyoma were73.8%,81.8%,44.4%,84.4%,40.0%.The five items of HS in the diagnosis of submucous myoma were 95.2%,97.0%,88.9%,97.0%,88.9%. Through the statistical analysis, the difference between the three methods had statistical significance(P<0.05).The difference between 3D-SIS and HS had no statistical significance(P>0.0125). By calculating the Kappa value between two of the three methods and between three methods with pathologic examination, we know that there were bad uniform for submucous myoma by TVS, HS and pathologic examination,there were good uniform for submucous myoma by 3D-SIS, HS and pathologic examination.Conclusion:1. The accuracy of 3D-SIS is higher than TVS in diagnosing submucous myoma.3D-SIS has higher value in the diagnosis of submucous myoma than TVS.2. The accuracy rate of 3D-SIS is close to HS in diagnosing submucous myoma and there are good uniform for submucous myoma by 3D-SIS, HS and pathologic examination. 3D-SIS can be used to provide information for clinical treatment in place of HS in appropriate cases.3. 3D-SIS is simple and safe in diagnosing submucous myoma. It has great prospects for development because it can provide accurate and detailed information which could provide important basis for clinicians to make a diagnosis and give treatment in patients.
