PartⅠ The value of measuring iodine concentration in perienteric adipose tissue with dual energy CT(DECT) for judgment of serosa invasion of colon cancer.Objective: To explore the value of iodine concentration in perienteric adipose tissue for the judgment of serosa invasion of conlon cancer.Methods : 53 patients of colon cancer confirmed by surgery and pathology in Shanxi Dayi hospital were selected during March,2015 to March,2016,and all patients underwent preoperative DECT.We transferred the data to the “Liver VNC”,a dual energy post-processing software.We measured the iodine concentration in perienteric adipose tissue by iodine map in arterial phase and venous phase.Then we made statistical analysis of iodine concentration in serosa invasion group and vninvaded group. ROC curve was used to evaluate the value of IC in determining serosa invasion.Results : The iodine concentration of serosa invasion group and uninvaded group in arterial phase was(0.5±0.1)mg/ml and(0.2±0.1) mg/ml,respectively(Z=-4.93,P<0.01).And the results in venous phase was(0.7±0.2) mg/ml and(0.3±0.2) mg/ml,respectively(Z=-5.52,P<0.01). With a cut-off of 0.35 mg/ml at arterial phase,area under curve(AUC) was 0.891,the sensitivity was 90.3%,specificity was 90.9%,positive predictive value was 93.3% and negative predictive value was 87.0%.With a cut-off of 0.45 mg/ml at venous phase,AUC was 0.945, the sensitivity was 87.1%,specificity was 90.9%, positive predictive value was 93.1% and negative predictive value was 83.3%.Conclusion: The iodine concentration in perienteric adipose tissue of colon cancer in sersora invaded patients was higher than who were uninvaded.The measurement of iondine concentration improved the accuracy of C T diagnosis of serosa invasion in colon cancer.PartⅡ The application value of virtual non-contrast(VNC) in colorectal cancer.Objective: To explore the application value of virtual non-contrast(VNC) in colorectal cancer.Methods : 73 patients of colorectal cancer confirmed by surgery and pathology in Shanxi Dayi hospital were selected during March,2015 to March,2016.The sanning program is same as PartⅠ.We obtained the VNC imaging by dual energy post-processing.In the CT imaging of true non-contrast(TNC), arterial phase VNC and venous phase VNC,we measured the CT values of the cancer focus, perienteric adipose tissue and lymph gland.And the software gene rated the noise automatically,then we calculated the signal-to-noise ratio(SNR) and contrast-to-noise ratio(CNR). Objective indicators of the three groups were statistically analyzed.We also made a subjective image quality scores for each patient.The radia tion dose between routine three phases and dual-energy two phases were compared.Results : The CT values of the cancer focus and lymph gland in arterial phase VNC and venous phase VNC were lower than the TNC. There was no difference between the CT values of perienteric adipose tissue in TNC and venous phase VNC(P=0.07).All the objective image quality index has no difference between arterial phase VNC and venous phase VNC. The two VNC subjective image quality scores were lower than the TNC,but the scores of all cases were above 3. The effective dose in dual-energy two phases was(16.05±2.04) m Sv,and in routine three phases was(21.51±2.41)m Sv, the difference was statistically significant.Conclusion: Dual energy virtual non-contrast of colorectal cancer had no effect on the diagnosis,and effective dose received by the patient is reduced by about 34.0%.It could be widely used in clinical examination of colorectal cancer...
