Objective:To probe the clinical effects and the related influencing factors of uterine fibroid treated with high-intensity focused ultrasound.Materials and Methods:1.45 cases(50 uterine fibroids) of patients with uterine fibroids that were diagnosed through clinical and imaging means and were treated with high-intensity focused ultrasound in Haifu Hospital, Chongqing from January, 2013 to December, 2014 were chosen.2.The volume changes of the fibroids before and after treatment were examined through magnetic resonance and ultrasound before HIFU treatment, on the 2nd day and in the 6th month after treatment, to observe the clinical effects as well as the occurrence of adverse reactions in the patients.3.Conducted single-factor and multi-factor analyses on the treatment effectiveness of HIFU of the ablation rate of different fibroids, ages, depth and types of fibroids, abdominal thickness, and the signal types of thefibroids on MRI-T2 WI with an aim to probe the related factors that influenced the effectiveness of uterine fibroids treated with HIFU.Results:1.45 cases of patients showed no severe adverse reactions during treatment and there was no adverse reaction 6 months after treatment, with1 being pregnant. The average volume of the fibroids in the 45 patients after a 6-month treatment reduced from(93.33±69.37) cm3 to(49.92±41.12)cm3, with the majority of relevant symptoms being improved: the improvement rate of heavy uterine bleeding was 78.3%(18/23), and one case showed aggravated symptom; and those of pelvic pain as well as pressure symptom were 55.6(5/9) and 100%(5/5), respectively.2.The treatment effectiveness of HIFU on uterine fibroids was affected by the ablation rate of fibroids and the signal types of the fibroids on MRI-T2 WI, but it had no distinct correlation with age, fibroid types and depth, as well as abdominal thickness. The single-factor analysis of the 50 fibroids suggested that the treatment effectiveness of HIFU was lower in fibroids with ablation rate <50% than in those with ablation rate ≥50%(P< 0.05); that was higher in fibroids with depth <8cm than in those with depth ≥8cm(P < 0.05); and that was higher in fibroids with low T2W1 signal than in those with equal and high T2W1 signal(P<0.05); while age,fibroid types and abdominal thickness displayed no distinct correlation with the treatment effectiveness of HIFU. Multi-factor analysis indicated that theablation rate of fibroids, as well as the signal types of the fibroids on MRI-T2 WI was the independent influencing factors of the treatment effectiveness of HIFU, while the fibroid depth was the confounding factor.Conclusion:1.High-intensity focused ultrasound, an effective and safe treatment method, had remarkable effects on treating uterine fibroids and brought no severe adverse reactions for patients, thus it was worthy of extensive clinical promotion.2.The treatment effectiveness of high-intensity focused ultrasound was affected by the ablation rate of uterine fibroids, as well as the signal types of the fibroids on MRI-T2 WI.
