Objective: To investigate the peripheral blood of neutrophil to lymphocyte ratio(NLR) levels in patients with intracranial aneurysm(IA) and to assess the risk of IA with the combination of Hunt-Hess grade and NLR.Methods: In the analysis, a total of 220 cases out of 1630 patients in our hospital from January 2014 to May 2015 were selected in the study and were divided into two groups:127 in aneurysmal subarachnoid hemorrhage(a SAH) group and 93 in unruptured intracranial aneurysm(UIA) group according to rupture. All the patient’s clinical information was recorded in detail and The severity of IA was preliminarily evaluated using the Hunt-Hess grade(0-Ⅴ grades) by s neurosurgeon. White blood cell(WBC) and its subtypes were obtained from patient’s venous blood. NLR was computed by taking the absolute neutrophil count and divided by the absolute lymphocyte count. NLR in the six Hunt-Hess grades were measured. Difference between groups was tested using Mann–Whitney U test. Correlation between WBC and Hunt-Hess grade as well as NLR,NLR and Hunt-Hess grade was analyzed by Spearman’s rho correlation analysis using SPSS20.0, respectively.Results: Clinical characteristics were comparable between the two groups(P>0.05). The count of WBC in the a SAH group was much statistically higher than that in the UIA group[11.95(8.78,16.24)×10~9/L Vs.5.82(4.74,7.00)×10~9/L,P=0.000];the absolute neutrophil count in the a SAH group was much statistically higher than that in the UIA group[10.48(7.24,14.47)×10~9/L Vs.3.57(2.73,4.54)×10~9/L,P=0.000], and also NLR in the a SAH group was higher than that in the UIA group [11.39(7.30,16.04)Vs.2.03(1.57,2.70),P=0.000],while compared to that in the UIA group, the count of absolute lymphocyte in the a SAH group was much lower[1.00(0.75,1.36)×10~9/LVs.1.68(1.36,2.06)×10~9/L,P=0.000]. Compared with that in Hunt-Hess 0 grade, NLR in Ⅰ-Ⅴ grade were much increased. And NLR in Hunt-Hess Ⅲ-Ⅴ grades were higher than those in Ⅰ-Ⅱ grades, respectively(P<0.05).WBC was positively correlated with Hunt-Hess grade(r=0.787,P=0.000) also positively correlated with NLR(r=0.799,P=0.000);NLR was positively correlated with Hunt-Hess grade(r=0.768, P=0.000).Conclusion: NLR might be associated with the rupture risk of IA; combination of Hunt-Hess grade and NLR might assess the severity of a SAH and predict its prognosis much more accurately.
