Objective:To investigate the incidence of stoma related complications and the artificial anus’s ability to control defecation through extraperitoneal route and intraperitoneal route by laparoscopic Mile’s surgery.Methods: 113 patients undergoing laparoscopic Mile’s operation in our hospital from June 2011 to June 2014 were selected as the research subjects and randomly divided into the observation group(stoma through extraperitoneal route,54 cases,3cases of loss to follow up)and the control group(stoma through intraperitoneal route,59 cases,8 cases of loss to follow up). Then the measures of telephone,letter,QQ,Email,outpatient,etc.were adopted to compare the occurrence situation of postoperative stoma complications and the difference of the ability to control defecation at postoperative 6,12 months.Results: The incidence rate of postoperative parastomal hernia in the observation group was largely reduced compared with the control group, the difference was statistically significant[0.0%(0/51)vs 1 3.7%(7/51),χ~2=5.522;P=0.019].However, the incidence rate of stomaedema in the observation group was significantly higher than that in the control group,the significant statistical difference also existed between the two groups[19.6%(10/51)vs.5.9%(3/51),χ~2=4.320,P=0.038].After half a year,the artificial anus’s ability to control defecation had no statistically significant difference between the two groups(P>0.05).However,after 1 year follow up, the artificial anus’s ability to control defecation and the excellent and good rate of comprehensive evaluation in the observation group were significantly higher than those in the control group, the differences were statistically significant[66.7%(34/51)vs.29.4%(15/51),P<0.05].Conclusion: Stoma through extraperitoneal route in laparoscopic Mile’s surgery can siginificantly decrease the occurrence rate of parastomahernia, has better ability to control defecation, so which is the first choice of stoma method in clinic.
