Objective:To learn the prevalence status of attempting smoking, current smoking, secondhand smoke exposure and explore their influencing factors among middle school students in Henan in order to provide scientific baseline data and evidence for corresponding tobacco control policy and measures. Methods:7789 middle school students from 16 districts in Henan province were chosen by stratified cluster sampling methods and investigated with a questionnaire. Database was established by Epidata 3.1. SPSS20.0 was used to weight, manage and analyze data. Results: 1 The epidemic status of tobacco useThe rate of attempting smoking of middle school in Henan was 23.8%, and the current smoking rate was 7.5%. Both rates of boys were higher than girls(χ2=99.076,P<0.001;χ2=89.911,P<0.001). Rates in rural areas were higher than urban areas(χ2=5.862,P=0.016;χ2=5.138,P=0.023), Besides, these two rates increased with age(χ2=65.473,P<0.001;χ2=19.838,P<0.001). 2 The epidemic status of secondhand smoke exposureThe rate of secondhand smoke exposure among middle school students in Henan was 72.4%. The rate of boys was higher than girls(c2=57.028,P<0.001). It is also higher in rural areas than urban areas(c2=4.956,P=0.033). This rate increased with age(c2=12.397,P<0.001). The rates of secondhand smoke exposure were different in different places. Indoor and outdoor public places were the highest(57.0%), followed by homes(43.1%) and public transport(15.8%). 3 The relevant influencing factors of attempting smoking, current smoking and secondhand smoke exposureResults of multi-factors Logistic regression analysis showed that the risk factors associated with attempting smoking included being boys(OR=4.250), age(OR=1.561; OR=1.810), being exposed to tobacco advertising and promotion(OR=1.221), peers’ smoking(OR=4.365), seeing teacher smoking at school(OR=1.449) and being able to buy cigarettes near schools(OR=2.413). The protective factors included considering smoking reducing attraction(OR=0.778), considering smoking in social occasions making people uncomfortable(OR=0.683), gotting smoking control information by the media(OR=0.765) and learning the dangers of tobacco use in classrooms(OR=0.703).The risk factors related to current smoking included being boys(OR=6.463), peers’ smoking(OR=13.887), seeing teacher smoking at school(OR=1.661), being able to buy cigarettes near schools(OR=2.750), considering smoking making people more attractive(OR=1.391) and considering smoking in social occasions making people more comfortable(OR=1.914). The protective factors included considering smoking making people less attractive(OR=0.699), considering smoking in social occasions making people more uncomfortable(OR=0.367) and getting smoking control information by the media(OR=0.773). The risk factors related to secondhand smoke exposure included being boys(OR=1.332), fathers’ smoking(OR=2.261), mothers’ smoking(OR=2.049), peers’ smoking(OR=2.466) and seeing teacher smoking at school(OR=2.525). The protective factors included rural areas(OR=0.596) and considering secondhand smoke must unhealthy(OR=0.770). Conclusion:1 The rates of attempting smoking and current smoking among middle school students in Henan were both higher than average level in China.2 The rate of secondhand smoke exposure among middle school students in Henan was higher, and the status exposed in public places was serious.3 The influencing factors accociated with smoking and secondhand smoke exposure were complicated. Peers’ smoking was the most important risk factor while the correct understanding of the dangers of tobacco was a very important protective factor.
