Total budget is a tool of controling medical costs from the supply level. Compared to other methods of payment, total budget was acclaimed due to the effect of having the best control. Since 2002 Shanghai has carried out the total budget and soon fully extended to all public hospitals. However, the system has controlled costs, it was facing many difficulties. Therefore, it is necessary to assess and amend the implementation of Shanghai’s total budget, in order to accommodate the people’s ‘new medical reform’ needs.This thesis is divided into four parts, seven chapters. Chapter I and II is the first part, it is the whole lay of the research. The opening introduces the background and significance of topics, research framework and methods. Through the second chapter of the relevant concepts, review the theoretical support and research combing, it determines on the research direction of the paper. The second part is divided into chapter III and Chapter IV, it is a specific statement of Shanghai total prepaid practice development and institutional evolution since the pilot, and the insight into the perception survey system from both doctors and patients in the positive experience in the implementation and operation of the status quo. The third part is the fifth chapter, it mainly analyzes the implementation from the concept dimension. Based public policy assessment standards and systems designed, there is a comprehensive assessment from system effects(cost control, the level of benefit), the implementation of the effect(the insured person’s response to the quality of medical services, resource allocation fairness) and efficiency of resource allocation in three dimensions. And then it exposes the institutional obstacles and problems. The fourth part is the sixth and seventh chapter. Based on the above policy approaches and quantitative deductionmodels, combined with typical country comparative analysis of total prepaid system, sum up the uniqueexperience which can be replicated to other areas.At last, put forward a specific strategies and suggestions on the benchmark of actual situation of Shanghai’s medical insurance.The innovation is from the macro policy perspective, the perspective of microscopic perception survey and assessment system to reasearch. It evaluated the system in-depth anatomy through the policy evolution and empirical assessment of total budget.
