Objective:1.To evaluate the expression of DNAJB1 and DNAJB6 which both are members of DNAJB families in NSCLC tissues.2.To confirm DNAJB1 and DNAJB6 expression in FOB(fibre optic bronchoscopic brushing)samples from lung cancer patients,and analyze the correlation with clinicopathologic parameters,such as age,smoking history,tumor type,tumor stage,tumor differentiation,gross pathology,and lymphatic invasion,analyze whether DNAJB 1 and DNAJB6 can improve the efficiency of cytological diagnosis,and investigate their diagnositic values in lung cancer.Methods :1.From January,2012 to March,2015,We chose 27 operative excisional cancer tissues and paired-adjacent tissues from NSCLC patients who did not receive any treatment for tumors including radiotherapy,chemotherapy and targeted therapy,and 6 cases of tissue samples of patients with benign pulmonary disease in the Department of Thoracic Surgery of The Third People’s Hospital Of Chengdu.15 cases of squamous cell carcinoma,11 cases of adenocarcinoma,1 case of glandular squamous cell carcinoma,4 cases of pulmonary tuberculosis,2 cases of inflammatory pseudotumor are involved.Then followed by random double-blind principle,respectively detected DNAJB 1 and DNAJB6 expression in various tissues samples by immunohistochemical techniques(Immunohistochemistry,IHC).Established the scoring disciplines,according to relevant documents and the mature IHC scoring system recommended by FDA from USA(DACO,HerceTestTM Interpretation Manual).Calculated the positive expression rates,and analyzed the correlation with the clinicopathological parameters.2.From April,2014 to January,2015,89 FOB samples Were collected from inpatients of The Third People’s Hospital Of Chengdu,and divided into two groups(lung cancer group and benign lesions group)according to the "gold standard" pathological diagnosis and clinical parameters.There were 37 cases of NSCLC,13 cases of SCLC in the lung cancer group and 39 cases in the lesions group.DNAJBl and DNAJB6 protein expression was deteded in FOB specimens through immunocytochemistry(ICC)technique,and analyzed the correlation with clinicopathological paramekrs.Results;1.DNAJBl was mainly located in nucleus.92.6% cases in the NSCLC group were confirmed positive expression of DNAJBl,which was significantly higher than that in benign group(16.7%)and paired-adjacent normal group(0%),P<0.01.There were no difference between benign group and paired-adjacent normal group,P>0.05.2.DNAJB6 was mainly located in nucleus.59.3% cases in the NSCLC group were considered DNAJB6 positive expression,which was apparently higher than that in benign group(0%)and cancer-adjacent normal group(0%),P<0.01.3.In 89 FOB samples from patients,DNAJBl was positive in 51.35% of NSCLC patients and 76.92% of SCLC patients,respectively,which obviously differed from that in benign group(5.13%),P<0.01.There were no statistical differences between NSCLC group and SCLC group,P>0.05.In 89 FOB samples from patients,DNAJB6 was positive in 45.95% of NSCLC patients and 61.54% of SCLC patients,respectively,which were significantly higher than that in benign group(7.69%),P<0.01,and there were no statistical differences between NSCLC group and SCLC group,P>0.05.4.In FOB specimens of lung cancer patients,the expressions of DNAJBl and DNAJB6 were not correlated with any clinicopathological parameters.5.The sensitivities of DNAJB1,DNAJB6 and bronchial brushing cytological morphology were 58.0%(29/50).48.0%(24/50)and 62.0%(31/50),respectively,and there were no statistical differences among them,P>0.05.Furthermore,three detective methods combined together could apparently improve the sensitivity of 88.0%,significantly higher than that of any single test,with high specificity of 84.62%(33/39),accuracy of86.52%(77/89),positive predictive value of 90.70%(39/43),negative predictive value of 76.09%(35/46).Conclusions:1.The expression of DNAJB1 and DNAJB6 was mainly located in nucleus.Significantly higher positive in lung cancer tissues than those in benign lung disease tissues and paired-adjacent normal tissues.2.In minimal invasive FOB specimens,DNAJB1 and DNAJB6 expression was also significantly higher positive in lung cancer than those in benign lung disease.3.The diagnostic value of DNAJB1 and DNAJB6 expression was equal to bronchial brushing cytological morphology.Combination detections of DNAJB1 and DNAJB6 expression and cytological morphology could significantly improve the sensitivity.
