| Object:In this paper, the major depressive disorder was taken as a study case. Under the condition that acupuncture and moxibustion therapy had exerted a definite effects on it, then adopted the way of "soothing liver, regulating heart, warming yang, replenishing energy" to cure the major depressive disorder. By means of psychological assessment, near-infrared spectroscopy cerebral function monitor and any other subjective and objective cross-references and combination, paid a detailed attention to the patients’ difference of depressive degree after the treatment of acupuncture and western medication, as well as made a relevant analysis of the cerebral function change so as to explore the efficiency and mechanism of acupuncture treatment.Content and Methods:Adopts the prospective study method, which divided the 16 standard patients into two groups randomly.They were the acupuncture and western medication group respectively.Using DSM-V depression’s diagnosis criteria, PHQ-9 scale and HAMD-17 item as a screening tool, HAMD-17 item as the assessment tool of severity, all of patients were conducted HAMD-17 evaluation both when the group was initially into being and after 1st,2nd,4th and 8th week of treatment.The course of treatment is eight weeks, and a total of five times to fill. At the same time, using near-infrared spectroscopy as a detection tool, detects patients’brain-related regions at first and after 1st,2nd,4th and 8th week of treatment. After completion of the treatment, separately evaluates the correlation between acupuncture and western medication from three aspects, i.e., the intervention effect on patients,the severity change of depression degree, as well as the brain-related region’s oxygen content change.Result:1. After the treatment, the HAMD scores of both groups were significantly lower so that there has no significance for statistics. (p> 0.05).2. The analysis of two groups’near-infrared spectral data after and in the process of treatment.The content of HbO2 of pre-frontal and dorsolateral cortex area showed a different degree’s increase, which has a statistic meaning. (p<0.05).Conclusion:1. Both the acupuncture and western medication treatment can improve depressive symptom of the patients. The two treatments have similar clinical efficacy.2. After the treatment, patients’ HbO2 levels in the brain depression-related area of acupuncture group and western medication group are raised to different levels. Changes of HbO2 in dorsolateral prefrontal cortex at the end of the 1st week of treatment are more significant in acupuncture group than in western medication group, while the changes at the end of the 2nd week and the 8th week are more significant in western medication group than in acupuncture group. Activation degree of depression-related area at the end of the 1 st week is stronger in acupuncture group than in western medication group, while the activity intensity at the end of the 2nd week and the 8th week is more significant in western medication group than in acupuncture group. Based on these characteristics, combination of TCM and western medicine can accelerate the onset time of clinical treatment.3. The acupuncture treatment for depression can improve the patient’s depression mood and increase HbO2 levels in emotional centers of depression patients, thus improving regulation ability on emotional centers and relieving clinical symptoms of depression. This may be the central mechanism of acupuncture for treating depression. |
