Influence Of Hyperbaric Oxygen Therapy On Peri-implant Tissue Healing:A Animal Study

Objective:To evaluate the effect of hyperbaric oxygen therapy(HBO) on peri-implant tissue healing in beagle models,this paper explores the mechanisms for hyperbaric oxygen therapy to promote early osseointegration and soft tissue healing, providing the reference for the hyperbaric oxygen therapy soft tissue defects in oral cavity.Methods:Part 1: Select 8 twelve-months old male beagle dogs which were randomly divided into 2 groups,respectively hyperbaric oxygen therapy group and blank control group,with 4 in each group.The third premolar in both quadrants of the mandible were used. Mucoperiosteal flaps were elevated and minimally invasive removed the mesial and distal roots. Planting the first postoperative day, the group placed in hyperbaric oxygen hyperbaric oxygen cabin animal hyperbaric oxygen therapy(turn off the hyperbaric chamber door slowly boost 15 min, rose 2.4 ATA, the regulator 60 min, buck 15min),1 day 1 time, 1 week 5 days, for 6 weeks.The blank control group is place in ventilated place under ordinary pressure.4 and 8 weeks after the surgery,two beagle dogs of the hyperbaric oxygen group and the blank control group were killded, remove the mandibular with implant, and examine the implant with X-ray, and CBCT. Measuring the implant stability quotient(ISQ) of the implant. The left mandible specimens of all beagles are made to be not decalcified hard tissue slice, and dyed with the methylene blue line-acid fuchsin for histological observation, and then did bone morphometric quantitative measurement analysis;the right mandible specimens are made to be decalcified paraffin sections,and dyed with HE,Masson.Part 2:Select 8 twelve-months old male beagle dogs which were randomly divided into 2 groups with 4 in each group, namely the hyperbaric oxygen therapy group and blank control group respectively.The hyperbaric oxygen therapy is the same as described in Part 1.Using the 5mm tissue cutting drill to cut the front,middle and back part of the hard palate to prepare round full thickness defect of soft tissue.Results:Part 1:(1) Clinical examination revealed eight experimental dogs implant all succeeded. Four weeks after the surgery,the average ISQ values ofthehyperbaric oxygen group was higher than the blank control group,and the difference was statistically significant(P<0.05).Eight weeks after the surgery, the difference was not statistically significant(P> 0.05).(2) Imaging showed that after eight weeks density of peri-implant was higher than those at 4 weeks. Hyperbaric oxygen group has less transmissive image.And trabecular bone is more orderly arranged.(3) Decalcified specimens were stained with HE and Masson staining, which showed that after four weeks of surgery, hyperbaric oxygen group have new bone formation;the control group saw a large number of necrotic cells and a small amount of bone osteoblasts.Eight weeks after surgery,hyperbaric oxygen group showed haversian bone plate;blank group showed more immature bone,and haversian bone plate is not formed.(4) Undecalcified section of methylene blue-acid fuchsin staining showed that four weeks after surgery hyperbaric oxygen group see new bone formation;blank group shows that gaps were formed between the deep blue bone cells,fibrous tissue, new bone tissue and the implant.There were less bones among the blank group.(5) statistical analysis shows that 4 and 8 weeks after surgery, bone-to-implant contact(BIC)and the percentage of bone area(BA%) of the hyperbaric oxygen group were higher than the control group.Part 2:(1) General observation: After three days, inflammation reaction of the hyperbaric oxygen group is less severe compared with the control group. 7 days after surgery, hyperbaric oxygen group healing areas were larger than that of the control group. 14 days after the surgery, the substrate was all healed. The epithelia of the hyperbaric oxygen group was integrate and that of the blank group shows etat lacunaire;(2) Histological observation: wound inflammation score and collagen fiber average optical density value results showed on days 3 and 7, hyperbaric oxygen group is better than the control group(P <0.05); on days 14,hyperbaric oxygen group and control group inflammation score was no significant difference(P> 0.05).(3) Immunohistochemistry Results: on days 3 and 7 and 14, CD31 mean optical density value of the result of hyperbaric oxygen group were higher than the control group, the difference was significant difference(P <0.05).Conclusion:In this study, the establishment of immediate implant and soft tissue defects shows that the bone forming effect and the soft tissue healing of the hyperbaric oxygen group are both better than that of the blank group. The reason is possibly that the hyperbaric oxygen group regulates various biochemical reactions involved in the stages of the peri-implant tissue healing.Therefore,in one hand,the time of the healing process was shorten,in the other hand, soft tissue-implant biologic seal was accelerated.The results suggeste that the clinicians can make the loading time in advance by using the hyperbaric oxygen healing after dental implants.
