Clinical Studies Of Xueduqing Intervene Sepsis Associated Encephalopathy Induced By Severe Sepsis

1 Objective Observe the intervention effect of traditional Chinese medicine xueduqing clinical studies in patients of severe sepsis induced septic associated encephalopathy, Investigate the clinical efficacy by observing inflammatory factors and symptom scores.2 Methods Admitted to our hospital from July 2014 to January 2016, 60 case patients diagnosed as severe sepsis induced septic associated encephalopathy were selected and randomly divided into 2 groups.The control group reference to the 2008 "surviving sepsis campaign"(SSC) guidelines for treatment, mainly including treatment of the primary disease(such as to remove the source of infection), the positive fluid resuscitation, the application of antibiotics, organ function support and protection(mechanical ventilation,hemofiltration,application of vasoactive drug,etc).Treatment group on the basis of conventional western medicine therapy plus xueduqing nasogastric, 100ml/times,2times/day, 7 days for a period of treatment.All medicines would come from the First Affiliated Hospital of Anhui college of traditional Chinese medicine,and made by the hospital tisanes chamber, a dose of a bag,100ml/bag.During treatment, the two groups were given the same nutritional support program.Compare symptom scores,peripheral blood WBC count,neutrophil count, C-reactive protein,procalcitonin,level of IL-1,lactic- acid,APACHE II ratings before and after treatment and other indicators of change, recording two groups of Glasgow score.3 Results(1) two groups of the WBC、NEUT were significantly decreased after treatment(P <0.05); Between the two groups after treatment, the treatment group the WBC、NEUT and CRP decreased more significantly,with statistical difference( P < 0.05).(2) PCT,IL-1 in both groups were significantly decreased after treatment(P < 0.05); Between the two groups after treatment, treatment group of PCT, IL- 1 reduce more apparent,with statistical difference(P < 0.05).(3) CRP,lactic-acid in both groups were significantly decreased after treatment(P < 0.05); Between the two groups after treatment, treatment group of CRP,lactic-acid reduce more apparent,with statistical difference(P < 0.05).(4)Two groups the patients APACHEII、GCS score before and after treatment a significant difference(P < 0.05), and comparison between groups has significant difference after treatment(P < 0.05).(5) Two groups the patients TCM symptom score before and after treatment a significant difference(P < 0.05), and comparison between groups has significant difference after treatment(P < 0.05).(6) In the treatment group,14 cases of clinical control, 8 cases of significant effective, 5 cases of effective,3 cases of ineffective, the total effective rate is 90%, in the control group,9 cases of clinical control,7 cases of significant effective,4 cases of effective,10 cases of ineffective,the total effective rate is 66.66%,the difference was statistically significant(P<0.05).4 Conclusion The intervention of Xueduqing can reduce peripheral blood WBC count, neutrophil count,and serum CRP, PCT, IL-1 levels,lactic-acid and improve patients’ inflammatory response;And can reduce the APACHE II、TCM syndrome score and improve Glasgow score value for the improvement of clinical symptoms,the restoration of organ function and illness recovery play a supporting role;The mechanism of action of xueduqing may be related to the regulate of the inflammatory response.
