Objective: To observe of Qingshen Granule on chronic renal failure(chronic renal failure, CRF) of damp heat syndrome in the peripheral blood of ROS,NF-?Bp65,p-I?B? protein in renal tissue from all groups. To investigate of Qingshen granule can NF-?Bp65 pathway mediated by oxidative stress in chronic renal failure patients with damp heat syndrome of body's intervention, reduce the clinical symptoms of patients with damp heat syndrome of chronic renal failure, improve renal function.Methods:60 cases of CRF patients with damp heat syndrome, Randomly divided into the treatment group and the control group of 30 cases(refer to random digital table),and another 20 cases of normal healthy physical examination patients(for detection of whole blood and serum samples). In the treatment process, 3 cases were excluded, 1cases in treatment group, 2 cases in control group, 57 cases in the final treatment group(29 cases in treatment group and 28 cases in control group). The treatment group was treated with traditional Chinese medicine enema, western medicine treatment, and clear kidney granule. The control group was treated with traditional Chinese medicine enema and basic treatment of Western medicine. The usage of clear kidney particles is: 3 times/ day, 1 bags / times(each bag granules 10 g, about including raw medicinal herbs 34g),the course of treatment is 12 weeks.Detection of two groups before and after treatment in patients with serum creatinine, serum creatinine, serum creatinine(SCR)) level, the expression of ROS?NF-?Bp65, p-I?B?, and to estimate the glomerular filtration rate estimated glomerular filtration rate(GFR), and to evaluate the two groups before and after treatment in patients with clinical efficacy and TCM syndromes.Results: 1 Treatment group total effective rate was 89.66%, 57.14% in the control group, differences in between groups(P<0.05).2 TCM treatment group, the total effective rate was 86.21%, 53.57% in the control group, the two groups had differences(P<0.05).3. Before treatment, treatment group and control group of TCM syndrome integral had no significant difference(P>0.05); compared with before treatment, after treatment, the treatment group of TCM syndrome integral, there is a significant difference(P<0.01);after treatment in the control group of TCM syndrome integral treatment before, there are differences(P<0.05). After treatment, control group and the treatment group of TCM syndrome integral compared the differences(P< 0.05).4.Before treatment,The treatment group and control group and the level of SCR and EGFR, there is no difference(P>0.05). SCR and EGFR than before treatment has significant difference(P<0.01), the control group improved is not obvious(P>0.05);treatment after treatment compared group and the control group and the level of SCR and EGFR(P<0.05).5. Before treatment, the treatment group and the control group of ROS?NF-?Bp65 and p-I?B? expression were significantly higher than those in normal control group(P <0.01). Before treatment, the treatment group and the control group of ROS?NF-?Bp65,p-I?B? expression had no significant difference(P>0.05). After treatment, the treatment group, ROS ? NF-k Bp65 and p-I?B? expression was lower than before treatment(P<0.01); after the control treatment group, compared with those before treatment improvement is not obvious(P>0.05); after treatment, ROS ? NF-?Bp65,p-I?B? expression was significantly lower than that of the control group(P<0.01).Conclusion: 1. The expression of peripheral blood ROS?NF-k Bp65 and p-I?B? CRF in patients with damp heat syndrome were increased, that oxidative stress mediated NF- activation of B signaling pathway play a role in the pathogenesis and progression of CRF;2.Qingshen granule can effectively relieve the clinical symptoms of CRF patients with damp heat syndrome, improve renal function;3.Qingshen granule could decrease the expression of CRF in peripheral blood of patients with ROS?NF-k Bp65 and p-I?B?, indicating activation of Qingshen granule could inhibit the oxidative stress mediated by NF-k B signal pathway;4.Patients in the treatment group were taken after Qingshen granule did not appear adverse drug reactions before and after treatment, no safety detection of abnormalities,this drug safety.Objective: The detection of Qingshen granules in high, medium and low three groups of the unilateral ureteral obstruction(UUO) induced renal interstitial fibrosis in rat kidney tissue SOD,ROS, MDA and nuclear factors-?Bp65(NF-?Bp65),p-I?B?,I?K?,E-cadherin(E-cadherin), alphasmooth muscle actin(?-SMA),Monocytechemotatic protein-1(MCP-1),Intercellular cell adhesion molecule-1(ICAM-1), protein expression of Qingshen granule in each dose; Set whether inhibition of UUO model of rats kidney tissue oxidative stress mediated by activation of NF-?B signaling channel, reduce the renal tubular epithelial cells transdifferentiation(EMT), anti renal interstitial fibrosis.Methods: Selected male SD 70 rats, 2 months old, clean grade, weighing 200 + 20 g, all purchases of from Beijing Weitong Lihua Experimental Animal Technology Co.Ltd.,closed management, adaptive feeding for 1 week. After 1 week by simple random sampling and were divided to 7 groups, including normal group, model group, Qingshen granule high dose group, Qingshen granule dose group, Qingshen granule low dose group, pyrrolidine dithio formic acid(pyrrolidine dithiocarbamate and PDTC) group and candesartan group with 10 rats in each. In addition to the normal group 10 rats,UUO model of the remaining 60 rats were prepared according to standard test method.Animals in each group were on the first day after modeling began to medicine, 1 times a day, each 2ml, Qingshen granule with high, medium, low dose group were given, clear renal granular aqueous solution by 4.0g/kg.d, 8.0g/kg.d and was 16.0 g/kg.d gavage.PDTC group will PDTC dissolved in warm water with 40mg/kg.d by intraperitoneal injection of [1]; candesartan group candesartan cilexetil tablets dissolved in warm water to 1.0mg/kg.d gavage; normal group and model group were given warm stomach, 1times a day, 2ml each time. Every 12 weeks. Biochemical method for determination of serum creatinine, serum creatinine SCR), 24 hour urinary protein(24-hour urine protein,24 h Upro); 2.Fenton reaction, enzyme measurement to detect renal tissue ROS, SOD,MDA content; 3. Immunohistochemical method was detected in kidney tissue NF-k Bp65, E-cadherin and alpha smooth muscle actin expression; 4. Western blot was detected in kidney tissue of NF-?Bp65, I?Ka, P-I?Ba, ICAM-1, and MCP-1 the protein expression; 5. Observe clear effects of renal particles of high, medium and low dose group of rat kidney tissue pathological study of.Results: 1.Comparison of in each group Scr, 24 h Upro: Compared with the normal group, model group, the SCR, 24 h Upro content increased significantly, and the difference is significant(P<0.01); and model group compared Qingshen granule high,low dose group, PDTC group, candesartan losartan, SCR, 24 h Upro content decreased, the difference is statistically significant(P<0.05); Qingshen granule with high, medium and low dose group compared middle dose group of SCR, 24 h Upro content minimum; Qingshen granule dose group of SCR, 24 h Upro content reduce and PDTC group, candesartan losartan group, the difference is statistically significant(P<0.05).2.Comparison of ROS ?MDA and SOD: Elevated levels of kidney kidney increased significantly compared with the normal group, the model group rats ROS, MDA and SOD level decreased significantly, the difference was significant(P<0.01); with the model group, the Qing granules with high, middle and low dose group, PDTC group,candesartan losartan, ROS, MDA levels were decreased, SOD levels were elevated, and the difference was statistically significant(P<0.05); Qing granules in high, middle and low dose group, middle dose group of ROS, MDA level decreased and the level of SOD increased, is better than that of the high and low dose group; Qing Shen Granule dose group of ROS, MDA levels decreased, SOD and PDTC group, candesartan losartan group, the difference was not statistically significant(P>0.05).3.Comparison of expression of NF- ?B p65, I?K?, p-I?B? protein in renal tissue from all groups(Western blot method): Compared with the normal group, model group, NF-?Bp65, I?K?, p-I?B? protein content increased significantly, there was significant difference(P<0.01); compared with the model group, Qingshen granules in high,medium and low dose group, PDTC group and candesartan group NF- ?B p65, I?K?,p-I?B? protein content decreased, the difference was statistically significant(P<0.05);comparison of Qingshen granules in high, medium and low dose group, middle dose group, NF- ?B p65, I?K?, p-I?B? protein content was the lowest, middle dose group NF- ?B p65, I?K?, p-I?B? protein content was lower than the high and low dose group;Qingshen granule in middle dose group, NF-?Bp65, I?K?, p-I?B? decreased the content of PDTC group and candesartan group, the difference was statistically significant(P<0.05).4.Kidney tissue of rats ICAM-1, MCP-1 protein expression were compared(Western blot): Decreased the content of kidney kidney significantly increased compared with the normal group, model group, ICAM-1, MCP-1 protein content, and the difference was significant(P < 0.01); with the model group, the Qing granules with high, middle and low dose group, PDTC group, candesartan losartan, ICAM-1, MCP-1 protein content decreased, the difference was statistically significance(P<0.05); Qing granules with high, middle and low dose group, middle dose group of ICAM-1 and MCP-1 protein content was the lowest, middle dose group of ICAM-1 and MCP-1 protein content was lower than the high and low dose group; Qing Shen Granule dose group of ICAM-1,MCP-1 and PDTC group, Kan candesartan group, the difference was statistically significant(P<0.05).5.Immunohistochemical E-cadherin staining: Compared with the normal group, model group of E-cadherin expression was significantly reduced, the difference is significant(P<0.01); and the model group compared Qingshen granule high,, low dose group,PDTC group, candesartan losartan group of E-cadherin expression were increased, the difference is statistically significant(P<0.05); Qingshen granule with high, medium and low dose group compared middle dose group of E-cadherin expression was the highest;Qingshen granule dose group of E-cadherin expression enhanced and PDTC group,candesartan losartan group, the difference is statistically significant(P<0.05).6.Immunohistochemical staining of a–SMA: Compared with middle dose group significantly increased compared with the normal group, model group of a-SMA expression, the difference was significant(P<0.01); with the model group, the Qing Shen Granule high, medium and low dose group, PDTC group, Kan candesartan cilexetil a-SMA expression decreased, the difference was statistically significant(P<0.05); kidney particles of high, middle and low dose group, middle dose group of a-SMA expression is the lowest, middle dose group of a-SMA expression was lower than the high and low dose group; Qing Shen Granule group of a-SMA expression decreased and PDTC group, candesartan losartan group, the difference was statistically significant(P<0.05).Conclusion:1. The oxidative stress mediated NF-k B signaling pathway activation in the formation process of UUO renal interstitial fibrosis.2.Qingshen granule can reduce the Scr, 24 h Upro UUO model rats.3.Qingshen granule can reduce ROS and MDA level in the kidneys of UUO rats,increase SOD level, reduce oxidative stress response in kidney tissue, improve the antioxidant capacity, and inhibition of NF-k B signaling pathway mediated by oxidative stress mediated, reduce inflammation, and improve renal tubular EMT, and to delay the progress of renal interstitial fibrosis.4.Pathological Qingshen granule can significantly reduce the renal tissue of UUO model rats to varying degrees, which leads to the inhibition of renal fibrosis;...
