ObjectiveTo explore the correlation between the carotid artery intima-media thickness (IMT) and cerebral infarction through using Color Doppler Ultrasonography to measure the carotid artery IMT in the patients with cerebral infarction or non vascular disease.Method160 patients with cerebral infarction in the Hefei Second People' Hospital were enrolled to this study from September 2014 to October 2014. According to the criteria of "Classification Method of the Cerebrovascular Disease" revised by the Fourth National Cerebrovascular Academic Conference of Chinese Medical Association in 1995 and "the Main Diagnostic Points of Cerebral Vascular Diseases", the 160 patients with cerebral infarction and with the exception of cardiac cerebral infarction, as test group, were finally validated and diagnosed by head CT or MRI scanning. At the same time,160 cases without cerebral infarction for physical examination or other patients as control group. All cases enrolled in the study were the local residents in Hefei city, or had lived here for more than three years; these cases'ages were over 50 years; they did some health consultation with the information about their blood pressure and the results of laboratory tests, and filled them in their questionnaire; Meanwhile, using the Philips IE33 High-Resolution Color Doppler Ultrasonic Diagnostic Instrument with the probe frequency of 3-11 MHZ. the carotid artery IMT were measured in the cases of test group and control group, all the above data were collected, and analyzed by SPSS version 17.0 software package, then explored the correlation between the carotid artery IMT and cerebral infarction.ResultsFirst, in this paper, we found that the carotid artery IMT in the test group were higher than that in the control group, and the difference between them was obvious statistically significant.Second, our data also showed that, in the patients of cerebral infarction group, such factors as ages, body mass index(BMI), diastolic blood pressure, blood calcium, urea nitrogen, total bilirubin, direct bilirubin, indirect bilirubin, and homocysteine were closely related with carotid artery IMT by simple linear regression analysis, and indicated that all the above factors were the risk factors of increased carotid artery IMT.Third, by the regression analysis, this study found that, in the patients with cerebral infarction, obviously correlation were presented between blood calcium and increased carotid artery IMT, while no obvious correlation was existed between gender and increased IMT; In terms of liver and kidney function and lipid metabolism test, obviously correlation were found between urea nitrogen and increased IMT, but no obvious correlation were showed between kalium, natrium, other electrolytes, and increased IMT; in addition, there were no obvious correlation found between creatinine, albumin, GGT, lipid metabolism and increased IMT.Fourth, according to the relationship between the carotid artery IMT and carotid atherosclerosis, IMT was divided into normal (less than 1.0 mm), atherosclerosis (less than 1.5mm; equal to or greater than 1.0mm), plaque formation (equal to or more than 1.5mm) three categories, with univariate analysis, data showed that the factors of age, BMI, blood calcium and blood urea nitrogen were closely related to carotid artery IMT, this results further illustrated that the above factors were the risk factors of thickened carotid artery IMT.Fifth, using multivariate analysis and adjusting for the effects of confounding factors, our data showed that age, blood urea nitrogen, BMT, calcium were the independent risk factors of thickening IMT and cerebral infarction, this results may explain the changes with the ratio 29.33%(P< 0.05).Sixth, in the control group, when cases' ages were less than 60 years old,40 cases were found with the normal of carotid artery IMT (76.9%t), and 12 cases with intima-media thickening or plaque formation (23.1%); when cases' ages were from 60 to 70 years old year old,26 cases were found with the normal of carotid artery IMT (50.0%), and 26 cases with intima-media thickening or plaque formation (50.0%) patients; when cases' ages were older than 70 years old,24 cases were found with the normal of carotid artery IMT (42.9%), and 32 cases with intima-media thickening or plaque formation (57.1%), and the difference was statistically significant among the above three groups. This data showed that, in old people, along with the age growth, the ratio of cases with intima-media thickening and (or) plaque formation has a rising trend.Conclusions the carotid artery intima-media thickness and (or) plaque formation were positively correlated with cerebral infarction.
