Factors Associated With Hemoglobin A1c Qualified Rate And Compliance Among Patients With Type 2 Diabetes Mellitus | | Posted on:2017-12-22 | Degree:Master | Type:Thesis | | Country:China | Candidate:L Y Xue | Full Text:PDF | | GTID:2334330485473245 | Subject:General medicine | | Abstract/Summary: | PDF Full Text Request | | Objective: The present study was to determine the status of glycemic control and identify factors associated with hemoglobin A1 c qualified rate and compliance among diabetic patients treated at primary health clinics.Methods: 278 patients were selected from Endocrinology specialist clinics in the Second Affiliated Hospital of Hebei Medical University.Data were collected from patients’ medication records,glycemic control tests and structured questionnaires.Multiple liner regression analysis was carried out to predict factors associated with hemoglobin A1 c rate and compliance.Results: We used the value of HbAlc as the index of disease control.The study found that the proportion of patients with good blood sugar control was lower than other published studies.The average HbA1 c is 8.0%.For self-management behavior among the patients,40.8% did not control their diet(sugar intake),55.3% did not engage in regular exercise and 88.9% did not smoke.Approximately 53.4% of the patients were compliant in taking their medication.We found that analyzed by multiple linear regression,variables associated with good glycemic control included age、 the drug utilization pattern、duration of diabetes mellitus and adherence of the patient to the treatment.Compared with the patients who were receiving a combination of insulin and oral antidiabetics,those receiving monotherapy and combination of oral antidiabetics were more likely to have good glycemic control.In the present study,the proportion of patients with good glycemic control was lower than that in other published studies.Older patients with a shorter duration of diabetes who were receiving monotherapy showed better glycemic control.Multiple linear regression analysis indicated that drug treatment compliance was positively associated with age and drug utilization pattern(P<0.05).There were 100 patients with good compliance,Compliance rate was 36.1%.The knowledge of diabetes in patients with multiple factor analysis was positively correlated with drug compliance.The level of compliance rate of exercise、dietary treatment and sugar monitoring were relatively bad.Conclusions:.HbA1 c qualified rate was 23.0%.The factors of influencing HbA1 c were age,duration of diabetes,treatment and compliance.Age and compliance were negatively correlated with HbA1 c value.The duration of diabetes and treatment were positively correlated with the HbA1 c value.and the duration of disease was negatively correlated with HbA1 c value.The influence factor of compliance were age and medication regimen.Age and medication regimen were positively correlated with compliance.The influencing factors of drug treatment compliance were diabetes knowledge and positive correlation.The rate of diet,exercise,blood glucose monitoring compliance were low. | | Keywords/Search Tags: | type 2 diabetes, Glycemic control, Hemoglobin A1c, Compliance, factors | PDF Full Text Request | Related items |
