Objective: To investigate the value of the improved Median nerve motor conduction velocity inspection in enhance the checkout rate of Ca rpal Tunnel Syndrome and early stage diagnosis.Methods: 80 cases patients were included in this study, between Ja nuary.2015~January.2016, this 80 cases of patients were diagnosed in ou r hospital as Carpal Tunnel Syndrome(CTS). In the 80 cases patients, there were 71 patients' electromyography(EMG) examine results show that the median nerve were abnormal of the far port; there were 9 patie nts' EMG examine results show that the median nerve were normal or the examine index were close to the critical value, but,they were confirmed as CTS by followed visit or operation. All the patients were checked by two groups, the tradition group and the improved group. To analysis the the value of the improved median nerve motor conduction velocity(MCV) inspection in enhance the checkout rate of CTS and early stage diagnosis. We use the conventional sensory nerve consequent test to inspect all the 80 patients, the results of the inspection were in relation to the diagnosis of the mild CTS group, but, this index can not do the analysis of the differences between the tradition group and the improv ed group. In addition, select 47cases(52 wrist) patients as the mild CTS group, these patients were diagnosed in our hospital as CTS, and the nerve conduction velocity(NCV) results show as mild CTS, mean that the median nerve' distal motor latency(DML)? 4.5ms[6]; Select 25 cases(31 wrist) healthy person that have the matching age and at the same time as the normal compare group; Analysis the DML, elbow-wrist MCV,wrist-palm MCV of the median nerve, and confirm the normal value ofmy laboratory( ±2.5s). Use SPSS13.0 statistics software to conduct the suitedtest and correlation study. To compare the rate of the count data, we use ?~2 test(we use the hypothesis test of matching design four tables data). we use ±s to said the measurementdata, to compare the m eans of the two groups, we use t test. when P<0.05, the results will be considered have statistics diversity, when P<0.01, the results will be considered have notable statistics diversity.Results:1 Median nerve MCV inspection results of the mild CTS group and the normal compare group1.1 Median nerve DML and wrist-palm MCV. These two indexes were considered have notable statistics diversity(P<0.01), mean that the median nerve DML were extended and the wrist-palm MCV were slowed in the mild CTS group.1.2 Median nerve elbow-wrist MCV. This index was considered have nonotable statistics diversity(P>0.01), mean that the median nerve elbowwrist MCV were not slowed in the mild CTS group.(Table 1)2 The normal value of median nerve MCV of my laboratoryAccording to the ±2.5s of the normal compare group, obtain that: DM L>4.00ms?wrist-palm MCV<39.00m/s?elbow-wrist MCV<46.00m/s are the abnormal measure of these indexes. Combine other's literature[18] and experience of reality operate, confirm that: elbow-wrist MCV<43.00m/s is the abnormal measure of this index.3 Median nerve SCV inspection results of the 80 observe objects62 cases were abnormal and 18 cases were normal, the positive rate was 77.50%. so, the checkout rate of CTS was 77.50%.(Table 2)4 Median nerve MCV inspection results of the 80 observe objects4.1 Median nerve MCV inspection results of the tradition group57 cases were abnormal in median nerve DML inspection, the positive rate was 71.25%. The elbow-wrist MCV were normal of all the 80 patie nts. So, the checkout rate of CTS was 71.25%.(Table 3)4.2 Median nerve MCV inspection results of the improved group69 cases were abnormal of the wrist-palm MCV, the positive rate was 86.25%. So, the checkout rate of CTS was 86.25%. The elbow-wrist MCV were normal of all the 80 patients.(Table 3)4.3 The checkout rates of CTS were different from the tradition group and the improved group.We use ?~2 test to compare the checkout rates of the two groups, thestatistics result was as follow: P=0.008. P<0.01, the result was cons idered have notable statistics diversity.(Table 4)Conclusion:The checkout rates of CTS were different from the tradition group and the improved group, the improved group was higher then the traditiongroup, there were notable statistics diversity between the two groups(P <0.01). Median nerve wrist-palm MCV inspection has important significance in the early stage diagnosis of CTS. When the clinical symptomsand signs are conform to the CTS, at the same time, the EMG inspection results show that the median nerve are normal or the examine in dexare close to the critical value, the improved median nerve inspection can explicit that yes or no there have a slow on the wrist of the medianmotor nerve, it can do the early stage diagnosis of CTS.In a word, the improved median nerve inspection have great practic alvalue in enhance the checkout rate of CTS and early stage diagnosis. Itcan provide important proof of the clinical diagnosis and the choice of treatments.
