Objective: To fix quantity left ventricular longitudinal movement and left ventricular twisting motion of coronary heart disease by using echocardiography velocity vector imaging technology,to analyze each measured parameter values of myocardial ischemia and left ventricular regional myocardial motion,quantitatively evaluate left ventricular regional systolic function.Methods: Chest pain,chest tightness,precordial discomfort highly suspected coronary heart disease patients in cardiology outpatient and inpatient treatment in Octoberr 2014 to December 2015 were selected,ECG and coronary angiography(coronary arteriography,CAG)diagnosed with coronary heart disease,a total of 39 cases,LAD stenosis <75% and ?50%were selected as group B,total of 19 cases,including 12 males and 7 females,ECG examination with ST segment flat,depression or T-wave inversion,cardiac biomarkers(c Tn1)were in the normal range;LAD stenosis ?75% were selected as group C(Including the left anterior descending artery stenosis of 99% to 100%)of 20 cases,12 males and 8 females,wherein the ECG showed ST-segment elevation,myocardial biomarkers(c Tn1)> 0.030 ng / ml diagnosed as acute myocardial infarction were total of 5 cases.Gender and age with group B,group C match of the normal 25 cases were selected as group A,physical examination,electrocardiogram,blood biochemical examination and conventional echocardiography were healthy,15 males and 10 females,group A,B,C group for the study were performed echocardiography,transthoracic ventricular apical four chamber,two-chamber,and parasternal long axis section basal segments,apical left ventricular short axis two-dimensional moving image was gathered,left ventricular long axis myocardial peak systolic velocity of 19 segments(Vs),strain(?),strain rate(SR)and 10 segments of left ventricular fractional heart twisting movement,the apex angle of rotation,rotation speed were got,and LV torsion angle so the overall motion information was counted.Effect on left ventricular myocardial ischemia and left ventricular longitudinal movement after twisting movement of coronary stenosis was evaluated,and conpared with coronary angiography results,to evelatue clinical value in VVI diagnosing coronary artery disease.Results:1?Left ventricular longitudinal movement between stages Vc from basal to apical segments segment of 3 groups showed a decreasing trend;3 Group ?,SR were negative;Left ventricular direction rotation of 3 groups was same,from cardiac twist in the same direction,as viewed from the apex to the bottom of my heart,bottom systolic heart was with a clockwise movement,the apex was with anticlockwise motion.2?Compared with group A,interval,anterior and apical Vs,?,SR of group C decreased significantly,the difference was statistically(P<0.05),was consistent with the left anterior descending artery blood supply range;Vs,?,SR of group B was no significant difference.3?Compared with the heart level of group A,front wall,systolic anterior septal angle of rotation,rotational speed of group C was reduced significantly(P<0.01),Group B was with no significant difference compared to group A;Apex level compared with group A,the rotational and rotaional speed of the front wall of the interventricular septum of group C group B reduced.4?Left ventricular twist angle of group C reduced most obviously,left ventricular twist angle was closely related to LVEF,positive correlation.Conclusion: VVI technology can quantitatively evaluate left ventricular and left ventricular longitudinal movement twisting movement,quantitatively evaluate coronary artery stenosis in patients with coronary heart disease with varying degrees of myocardial longitudinal direction of regional wall motion abnormalities twist and positive correlation between left ventricular torsion and LVEF,evaluate local and global systolic function of left ventricular.VVI technique is superior to conventional techniques,can be used as a noninvasive method,early,objective exclude important adjunct to coronary heart disease,provide an important basis for clinical diagnosis and treatment as soon as possible.
