| Objective: To investigate the effect and side effects of intra-articular with different doses of morphine combined with dexmedetomidine on postoperative after arthroscopic knee surgery.Methods: Sixty patients of ASAⅠ-Ⅱ,aged 18-65 years,wighted 45-85 kg,with general anesthesia unde rgoing arthroscopic knee suegery were divided into 3 groups at random in a double-blind placebo controlled study.Group D received intra-articular 50μg dexmedetomidine only,group M1 D received intra-articular 1mg of morphine combind with 50μg of dexmedetomidine and group M2 D received intra-articular 2mg of morphine with 50μg dexmedetomidine.The volume of the drug sulotion injected in each patient was 15 ml.Recorded pain visual analogue scale(VAS),postoperative 2 hours Ramsay sedation score,the time to first postoperative analgesic request,the total postoperative analgesic use during the first 48 hours and all the adverse effects.Results: The VAS scores in group M1 D and group M2 D was lower than group D for 8、12h after operation(P<0.05).There was no statistically significant difference amomg three groups from 24 to 48 h after operation(P>0.05).Group M2 D was lower than group M1 D for 8h VSA scores after operation(P<0.05)and there is no statistically significant difference between 12 h to 48h(P>0.05).The time to fist postoperative analgesic request was longer in group M1 D and group M2 D compared with group D(P<0.05).Total fiurbiprofen axetil requirement was significantly lower in group M1 D and group M2Dthan group D(P<0.05).The Ramsay sedation score at 2 h after operation had no statistically significant difference among three groups(P>0.05).Conclusion: Intra-articular morphine(1mg,2mg)combined with dexmedetomidine(50μg)can provid effective postoperative analgesia after arthroscopic knee surgery,prolonged the first time for postoperative analgesia and reduced the amount of postoperative pain relivers.In addition,2mg of morphine enhanced more effective analgesia than 1mg of morphine after operation. |
