Objective1.To confirm the efficacy about sleeve gastrectomy for the treatment of obesity and T2 DM in rats.2.To find an economic,security and effective approach that is the most physiological way to build diabetic rats.3.We explore the change of ghrelin secretion of rats after sleeve gastrectomy,and study its possible role in diabetes remission.4.By changing the feeding rhythm reasearch how sleeve gastrectomy changes the rhythm of ghrelin secretion.Methodwe made a formula for the high fat and sugar feed by ourselves,after review many literatures,feeding this food after 12 months,successfully we built 23 obesity and T2 DM rat models from 50 rats.They were randomly divided into two groups,one group is sleeve gastrectomy group(SG),the other group is control group(S).After surgery fasting for 2 days liquid diets for 3days and then became normal diet,the S group is as the same.respectively,1 week,4 weeks,8 weeks after surgery when fasting orbital venous blood.Detection of AG,TG,insulin,blood glucose,glycosylated hemoglobin,computing HOMA-IR,AG/TG,AG/UAG index,analysis of the sleeve gastrectomy diabetes treatment effect.In addition from 8 weeks to change dietary laws,fasting began to hurl food three times a day after a day,divided into the early to middle three times,and half an hour at a time.Changing diet rules after January,SG group and S group respectively take blood before meals and after meals,AG,TG change detection.Observe the sleeve shape for the influence of ghrelin secretion rhythm after gastrectomy.ResultsThere is only one rats died after sleeve gastrectomy,it may be caused by abdominal infection.The rest of the rat has been alive until the end of the experiment.Our team after use high fat and sugar feed to build diabetic rats models which are very stable.Modeling rate is nearly 40%.The average weight of rats after 12 months high fat and sugar raising is more than 650 g,the average blood sugar is about 8.6mmol/l,the percentage of glycosylated hemoglobin is nearly 6.5%.HOMA-IR calculation value is about 6,far more than before modeling.And with high fat and sugar feeding rats AG rise from 100ng/l to 250ng/l,TG decreased from 1000ng/l to 850ng/l.And a corresponding increase in the proportion of AG/UAG.In brief we finally construct fatty and T2 DM rats model and surgical models successfully.The weight of sleeve gastrectomy group decreased significantly compared with control group after surgery,the average weight loss is more than 200 g,stable and without weight rebound.Sleeve gastrectomy is an ideal treatment for diabetes,within the first week,partly remission rates is nearly 60% of the diabetic rats(blood glucose below 5.6-5.6 tendency/l),after the fourth week,80% of rats with complete diabetic remission(restore normal blood sugar,the percentage of glycosylated hemoglobin is less than 6.5%).And the effect is very stable,until 8 weeks postoperative the percentage of glycosylated hemoglobin remained normally.The concentration of AG and TG decreased obviously after Sleeve gastrectomy,AG fell from 250ng/l to 100ng/l,TG fell from 850ng/l to 500ng/l,AG/UAG also decline obviously.The group of shame surgery is still in the condition of fatty and diabetes.And the concentration of AG,TG and AG/UAG basically remain unchanged in control group.The weight loss of compared rats in perioperative period may due to induced food intake.as is shown in the research,ghrelin secretion rhythm can be changed by altering dietary pattern,predietary changes of ghrelin is common in human being which can be induced in rats.Compared with the experimental group,AG changes similar to TG changes presents obvious predietary rhythm in shame surgical group,conversely in the experimental group AG decreases and changed bluntly in predietary.We find that changes of dietary rhythm affect the secretion of ghrelin.We make a bold assumptions,ghrelin secretion may be is controlled by the way of feedforward regulation.Sleeve gastrectomy restrains the activity of GOAT,keeping AG production at a low levelConclusion1.Rats were fed with high fat and sugar feed can induce T2 DM for a period of time,but time is different in different individual.2.Sleeve gastrectomy is a safe and effective treatment for obesity and T2 DM.3.Sleeve gastrectomy changes the concentration of AG and TG which may play an important role in diabetes remission.4.The secretion of ghrelin can be changed by altering the rhythms of diet.Sleeve gastrectomy can also change the rhythm of ghrelin secretion in rats.Obesity is closely related to the onset of T2 DM.Sleeve gastrectomy can be a safe and effective treatment for obesity and T2 DM,but the related mechanism is not clear yet.Ghrelin in which is still controversial about its function.We detailed in this study the rats from health to occur in the process of obesity and T2 DM,provides a very valuable animal experimental evidence.In this experiment,we found the changes of AG after sleeve gastrectomy in which was very supportive cummings professor’s theory.
