Trichosporon spp.are basidiomycetous yeast-like anamorphic fungi,which are widely distributed in nature and routinely isolated from clinical laboratories related to episodes of colonization or superficial infections,however,these fungi have also been recognized as emergent opportunistic pathogens causing invasive infections worldwide,mainly in immunocompromised hosts,and Trichosporon asahii(T.asahii)is the main pathogen.As the main type of invasive trichosporonosis(IT),Trichosporon fungemia has crude mortality rate as much as 53%-76%.Although recent European guideline of IT recommended triazoles as the first line drug for treating invasive Trichosporon infections.T.asahii is less susceptible to amphoterincin B than other Trichosporon spp.Also,clinical T.asahii isolates with high MIC of Fluconazole(16 ?g/ml)has been reported.Some IT patients are difficult to be completely cured and easily relapsed,which may be caused by that Trichosporon spp.adhere to invasive medical devices to form biofilms which included structured microbial communities embedded in an extracellular polymeric substance(EPS),so the strains are markedly enhance resistance to antifungal agents and may escape from host immune responses on account of the three-dimensional physical structure.Limit selections of antifungals,long development cycles and high cost for new targets antifungals require an urgent need for conventional drug in new usage to develop novel antifungal strategy against IT.In our previous study,amphotericin B / caspofungin combination shows synergistic effects against clinical isolates of T.ashii.Based on our earlier stage researches and related literatures,two types of conventional drugs were screened for the drug combinations which are non-steroidal antiinflammatory drugs(NSAIDs,included aspirin,ibuprofen and diclofenac sodium)and heat shock proteins 90-calcineurin inhibitors(geldanamycin and cyclosporin A).The MICs of those drugs and four antifungal drugs(Fluconazole,itraconazole,voriconazole,amphotericin B and caspofungin)were tested by the broth microdiliution method in 96-well plates(Costar)according to the Clinical Laboratory Standards Institute(CLSI)standard M27-A3 and checkerboard microdilution method.Though the inhibiting effects of all the five drugs were weak except geldanamycin,obvious synergistic results were founded for all 25 drugs combinations and the highest percentages of synergistic effects against T.asahii were found for amphotericin B with ibuprofen,cyclosporin A and geldanamycin,which suggest further in vivo/ animal study to test the potential effect of the NSAIDs and Hsp90-calcineurin inhibitors and further associated mechanisms need to be investigated.
