| ObjectiveThis study was designed to explore the effect analysis about three kinds of nasal feeding pump methods on ICU critical patients with enteral nutrition,,thus providing guidance for ICU patients to choose reasonable nasal feeding pump method.In order to reduce enteral nutrition complications,improve patients ’ condition and decrease nursing workload.MethodsA clinical randomized control study was conducted.Indwelling 120 critical patients who accept enteral nutrition therapy and were hospitalized in ICU of the fourth affiliated Hospital of Nantong University of the first people’s hospital in Yancheng from July 2014 to July 2015 were randomly assigned into three groups,which is the continuous 6-hour and intermittent 2-hour every three time pump group and continuous 24-hour pump group and intermittent 24-hour(continuous 2-hour and intermittent 1-hour every eight time units)pump group.Three groups carry out randomized controlled clinical trial study,after the nurse intervention,to observe the main index,such as complication and nutritional status,and secondary indicators,such as immunity and the change of patient’s condition.Using SPSS19.0 statistical software to analysis these data.Results1.Complications: the incidence of diarrhea and constipation among the three groups is not statistically significant(P>0.05),but the incidence of gastric retention rate is statistically significant(P<0.05).The pairwise comparison among the three groups,the gastric retention rate of the continuous 24-hour pump group is significantly higher than that of the continuous 6-hour and intermittent 2-hour pump group and the intermittent 24-hour pump group,the differences were statistically significant(P<0.05).There is no statistically significant difference between the continuous 6-hour and intermittent 2-hour pump group and the intermittent 24-hour pump group,(P>0.05).2.Nutritional status:there is no statistically significant difference among the three kinds of nasal feeding methods in albumin,prealbumin and transferrin(P> 0.05),but three groups of serum protein are reduced nasogastric the twelfth day in comparison with the first day.3.Immunologic function : there is no statistically significant difference among the three kinds of nasal feeding methods in the lymph node count(P> 0.05).The lymph node count is also no statistical significance between the first nasal feeding day and the twelfth day(P>0.05).4.Illness: there is no statistically significant difference among the three kinds of nasal feeding methods in the APACHE-Ⅱscore(P>0.05),but there is statistically significant difference between the first day and the twelfth day(P <0.05).Conclusions1.The three kinds of nasal feeding methods play the same role in prevention of diarrhea and constipation,but the continuous 6-hour and intermittent 2-hour and the intermittent 24-hour pump methods are slightly better than the continuous 24-hours pump method.2.Three kinds of nasal feeding methods play the same role in the maintenance of nutrition,but the nutritional indices of three groups is lower,suggesting that enteral nutrition should be begin as soon as possible.3.Three kinds of nasal feeding methods are able to maintain ICU patients’ s immunity.4.Three kinds of nasal feeding methods are able to improve ICU patients ’ condition.5.Compared with ICU class three a week and feeding times,the continuous 6-hour and intermittent 2-hour every three time pump method,on one hand,reduce the nursing workload each class each feeding,on the other hand,consistent with the dietary habits of our three meals a day on weekdays.intermittent 24-hour pump methoda,eight times a day,on one hand affect the patients’ rest,on the other hand increase nursing’ workload.Thus the continuous 6-hour and intermittent 2-hour pump method is more suitable for ICU patients. |
