Background Severe brain injury will bring huge economic losses and mental shock to patients and their families,and may formed a social problem.Brain death patients would be a source of organ transplant donor as the law currently recognized.The quality of organ transplant donor is highly concerned by modern scholars and acceptors.Nowadays,domestic and foreign scholars research little on functional changes of early prognosis of vital organs in the severe brain injury patients.Moreover,The studies of brain death focus on animals studies and less research on liver and kidney for the human brain after death.Therefore,we conducted research on such patients.Objective Investigating the organ function changes and prognosis in critically ill patients with brain injury of the Glasgow Coma Score(GCS)?8 points and brain death patients,aim to provide references with clinical treatment and protection for the organ function after brain death.Methods 32 patients with GCS score ?8 from 2014 October to 2015 April in Wuyang County People's Hospital were randomly selected into experiment group,and according to their liver function(in alanine aminotransferase(ALT)and aspartate aminotransferase(AST)as the main indicator),glucose,albumin,renal function(serum creatinine(BUN)and blood urea nitrogen(Scr)as the main index)and blood gas analysis on admission and after admission on the 1 day,2 days,3 days and 5 days,divided into survival group and death group were statistically analyzed.Retrospective analysis of 10 patients in Hospital of People's Liberation Army 371 from 2007 to 2008,and analyzed according to the announcement of alanine aminotransferase after brain death in different periods,aspartate aminotransferase,the first day after serum creatinine,urea nitrogen values on admission measured value.We analyzed data using the SPSS19.0 software for statistical analysis,observations are,measurement data,mean and standard deviation((?) ± S)are provided for continuous variables,calculate the correlation analysis,serious skewness data using non-parametric t-test and P < 0.05 was deemed statistically significant.Results There were significant differences in term of age,admission GCS score and random blood glucose between the survival group and death group in our study(P <0.05).In patients with severe brain injury,admission GCS score and random blood sugar on admission were significantly different(P <0.05)in the comparison group survival and death group.Admission level and quality of life of patients with GCS score was positively correlated(P <0.05).2.In patients with severe brain injury,the first five days of non-specific changes in liver function,but renal function changes were statistically significant(P <0.05).3.The random in patients with severe brain injury admission have relevance for the prognosis of patients and has a positive correlation with the mortality rate of patients,the dynamic changes of blood glucose is important in patients with severe brain injury,There were significant differences on dynamic change of blood glucose level between the survival group and death group(P <0.05).4.The mortality in patients of severe brain injury patients with metabolic alkalosis was significantly higher(P <0.05).5.There were significant differences in alanine aminotransferase values and aspartate transaminase changes,renal function(creatinine,blood urea nitrogen)changes between immediately after death in patients with brain death cereal and the first day admission in hospital(P <0.05),but they did not reach the value of severe injury,so prompt declared brain-dead after liver and kidney damage is not great.Conclusions 1.Severe brain injury patients age,GCS score,glucose and albumin levels in patients with exacerbation of symptoms and risk of death.2.Patients with severe brain injury early organ protection in favor of the treatment of them,and also conducive to increased symptoms organ protection cause brain death.
