Objective: The aim of the study is to explore the factors influencing the prognosis of the patients after surgery by retrospectively analyzing the survival rate of 343 cases of hepatocellular carcinoma(HCC) patients after hepatectomy.Methods: 343 cases of hepatocellular carcmoma patients who underwent the Hepatectomy between January 2011 to December 2014 in hepatobiliary surgery department of Hebei Medical University the fourth hospital were summarized retrospectively. The factors that may influence the prognosis of the HCC operations were selected, including 14 variables: age, sex, hepatitis B surface antigen, HCV antibodies, Cirrhosis, preoperative AFP, preoperative ALT, Child-Pugh grade, tumor size, tumor number, blood vessel invasion, intraoperative blood transfusion, TNM staging, BCLC staging. All data was analyzed using SPSS 17.0 software, Chisquare test was applied to analyze the Count data, and t test was applied to analyze the Measurement data. Kaplain-Meier method was applied to compare the survival rates. Univariate analysis was conducted using Log-rank test and Multivariate analysis was conducted using Cox's regression. P<0.05 was considered that difference have statistically significance.Results: Of all 343 cases, 226 cases(65.9%) are survived while 117 cases(34.1%) died. The 1-year, 3-year and 5-year overall survival rates were 86.3%, 64.1% and 54.8% respectively. Univariate analysis showed that the preoperative AFP(P=0.000), tumor size(P=0.000), blood vessel invasion(P=0.000), TNM staging(P=0.000), BCLC staging(P=0.000) were important risk factors affecting postoperative prognosis of hepatocellular carcinoma patients. Cox multivariate analysis showed that the preoperative AFP(P=0.003), tumor size(P=0.012) and BCLC staging(P=0.011) were independent risk factors influencing prognosis of HCC patients after hepatectomy.Conclusion: Studies of this group have shown that the AFP leve, tumor size, blood vessel invasion, TNM staging, BCLC staging were important risk factors affecting prognosis of HCC patients after hepatectomy. The AFP leve, tumor size and BCLC staging were independent risk factors.
