Objective:In this study,Compare transthoracic right heart contrast and color doppler flow imaging technology on the basis of Valsalva breathing to discover patent foramen ovale and right to left shunt caused by patent foramen ovale,analyze two methods of detection rate.It can improve the diagnosis of patent foramen ovale,providing a more reliable basis for clinical prevention and treatment.Methods:Select neurology hospitalized patients for the exclusion of other factors defined as cryptogenic stroke,and select volunteers from healthy management center matched with neurology hospitalized patients by sex and age group data.In quiet breathing and Valsalva breathing,respectively,in the right side supraclavicular fossa view,measured superior vena cava flow spectrum of S wave D wave,analyze data statistically,Whether it helps blood reflux by Valsalva breathing and increased right atrial pressure.Neurology patients at Valsalva breathing select color doppler flow imaging and right-heart contrast respectively,to observe whether the abnormal atrial level shunt.Contrast agent selected 9ml of 0.9% sodium chloride solution and 1ml air,placed in two 10 ml syringes,connected to the three-way switch,push back and forth 20 times to mix.So that the solution is milky white suspension.After rapid intravenous injection,observe the right atrium microbubbles developing and whether microbubbles across the atrial level.Results:(1)Select 32 Neurology cases patient,13 males and 19 females,The average age is 48.44±6.44 years old and 32 volunteers matched with neurology hospitalized patients by sex and age group data.(2)When the two groups Valsalva breathing inhale,the peak velocity of S wave?D wave than those during quiet breathing,and the difference was statistically significant.Whether Valsalva breathing and quiet breathing,the data between the two groups were not statistically significant.(3)In 32 Neurology cases patient,by color Doppler flow imaging detected 5 cases of patent foramen ovale positive,negative 27 cases.Transthoracic right-heart contrast detected 12 cases of patent foramen ovale positive,negative in 20 cases.Positive patients with Valsalva breathing,The left atrium was observed microbubbles,appearing right to left shunt at atrial level.Conclusion:(1)Valsalva breathing can make increase negative intrathoracic pressure,venous return rate and right atrial pressure,Through the superior vena cava flow spectrum can reflect the right atrial pressure indirectly.(2)Transthoracic right-heart contrast is an important addition to color Doppler ultrasound in diagnosis of patent foramen ovale and right to left shunt caused by patent foramen ovale,in favor of unexplained stroke further study,It has high clinical value.
