Clinical Study On Lamivudine And Adefovir Dipivoxil Combined With Qebaogangao In The Treatment Of Compensatory Stage Of Hepatitis B Cirrhosis (Liver And Spleen Deficiency)

Objective: The research's aim was to observe the efficacy of Lamivudine and Adefovir dipivoxil combined with Qebaogangao in the treatment of Hepatitis b cirrhosis compensatory period,and compared to the effect of the group which only took the Lamivudine and Adefovir dipivoxil to cure Hepatitis b cirrhosis compensatory period,then evaluation the efficacy of Lamivudine and Adefovir dipivoxil combined with Qebaogangao in the treatment of Hepatitis b cirrhosis compensatory period.Methods: The selected cases were from Ruikang Hospitol Affilited to Guangxi Universtiy of Chinese Medicine outpatient or inpatient department patients,Selecting 90 patients according to diagnostic criteria for hepatitis b cirrhosis compensatory period(Liver and spleen deficiency),randomly divided into two groups.One was observation group,and the other one was control group,each group contained 45 cases.The observation group took Lamivudine 100 mg one day at a time and Adefovir dipivoxil 10 mg one day at a time combined with Qebaogangao(milkvetch root,turmeric rhizome,baizhu,polyporus,salvia miltiorrhiza,bupleurum,curcuma root,zeeland,psyllium,bian banlian,schizandra)30g two times a day.Furthermore,the control group took Lamivudine 100 mg one day at a time and Adefovir dipivoxil 10 mg one day at a time.The treatment period was a total of 1 year.Before and after the treatment,observed and compare the patients' s TCM syndrome integral,liver function,HBV-DNA quota,HBV marker,serum indexes of liver fibrosis,abdominal ultrasonography,blood routine examination,blood routine examination,stool routine examination,renal function,ck and adverse reactions.Results:(1)TCM syndrome integral: The observation group and the control group of TCM syndrome scores were decreased after treatment(P<0.05).Compared with the control group,the TCM syndrome score after treatment in the observation group was significantly lower than that in the control group,and the difference between the two groups was statistically significant(P < 0.05).(2)Changes in the virus: The observation group and the control of the negative rate of HBV-DNA after treatment results showed no significant difference(P > 0.05).The observation group and the control group of Hbe Ag negative rate after treatment results showed no significant difference(P>0.05).(3)Main indexes of liver function: The observation group and control group of AST,ALT were decreased after treatment(P>0.05);The observation group AST,ALT after treatment compared with the control group after treatment,the difference was not statistically significant(P>0.05).(4)Serum indexes of liver fibrosis: In the observation group and control group,the levels of hyaluronic acid,type IV collagen,laminin and type III collagen were decreased before treatment(P<0.05).The observation group of hyaluronic acid,type IV collagen,laminin,type III after treatment,compared with the control group after treatment decreased significantly,the difference between the two groups was statistically significant(P<0.05).(5)Spleen thickness:The thickness of spleen in the observation group was smaller than that before treatment,and before and after treatment,the difference was statistically significant(P<0.05).There was no significant difference in the spleen thickness Before and after treatment in the control group(P>0.05).(6)Hepatic hemodynamics:(1)Hepatic portal vein:In the observation group the hepatic portal vein was reduced after treatment(P < 0.05).There was no significant difference in hepatic portal vein Before and after treatment in the control group(P>0.05).(2)Splenic vein internal: The observation group and the control group in the treatment of splenic vein were no significant change(P>0.05).Compared with the control group,the observation group had no significant difference compared with the control group(P>0.05).(3)Blood flow velocity of portal vein and splenic vein blood flow velocity: The observation group and the control group respectively in the portal vein blood flow velocity and blood flow velocity of splenic vein after treatment were better than those before treatment(P<0.05).The observation group of blood flow velocity of portal vein and splenic vein blood flow velocity,compared with the control group after treatment decreased significantly,the difference between the two groups was statistically significant(P<0.05).(7)Safety evaluation: There was no obvious change in the two groups of patients with blood routine,urine routine,stool routine,renal function,electrocardiogram,There was no significant difference in the comparison before and after treatment(P<0.05).Conclusion: Lamivudine and Adefovir dipivoxil combined with Qebaogangao in the treatment of Hepatitis b cirrhosis compensatory period(Liver and spleen deficiency)has definite therapeutic effect.In the improvement of TCM syndrome,serum liver fibrosis,hepatic portal vein blood in,blood velocity of portal vein,splenic vein blood flow velocity index is better than Lamivudine and Adefovir dipivoxil.And there was no obvious adverse reaction.
