| BackgroundOne of the most common complications of chronic kidney disease is hypertension.Inthe early stage of hypertensive nephropathy,micro albumin urine and proteinuria areappeared,then the renal function begins todecline,and renal failure occurs eventually.Elevation of serum beta 2MG reflect whether impaired glomerular filtration function or filtered load increased and?2-MG in urine reflect renal tubular damage or filtered load increase.The content of U-mAlb can reflect of kidney damage.It is a sensitive marker of glomerular injury in early.Therefore,we detected these three indicators,in order to improve the detection rate of early hypertensive nephropathy,this plays a key role in finding early hypertensive nephropathy in clinic.ObjectiveTo investigate the diagnostic value of combined detection of serum,urine beta2-MG and urinary albumin in patients with primary hypertension and renal disease.Methods1.Object of research,62 primary hypertension patients who were hospitalized in Department of nephropathy and Department of Cardiology of First Affiliated Hospital of Xinxiang Medical College from October 2013 to October 2015 were selected as experimental group,during the same period,45 healthy persons were selected as the control group.There was no significant difference in age and sex between the experimental group and the control group.2.Use clinic blood pressure to evaluate blood pressure.Specimen collection: In the early morning fasting venous blood 3ml,centrifuge5 minutes,determination of serum beta 2-MG,Scr,UREA,UA content,and the collection of urine 5ml,urine 2-MG,urine,U-mAlb content.3.To evaluate the value of serum and urine beta 2 micro albumin and urinaryal bumin in early diagnosis of hypertensive nephropathy.Results1)There were no significant difference(P>0.05)in age,the level of serum creatinine(Cr),uric acid(UA),blood urea nitrogen(BUN)between hypertension group and control group.2)In hypertensive group,the levels of serum beta 2 micro albumin,urinary beta 2 micro albumin and urinary albumin were significantly increased compared with the control group(P<0.05).3)U-mAlb results in Class I,II,III hypertensive patients were significantly higher than those of the control group and have significant statistical significance(P<0.05),U-mAlb levels in grade II and grade III hypertension patients were higher than that of grade I and have statistical significance(P<0.05),U-mAlb content in grade II and grade III high blood pressure patients have not any statistical significance(P>0.05).4)Blood,urine beta 2 microspheres protein in Grade II and grade III hypertension patients were higher than the level I and have statistical significance(P<0.05).blood beta 2 microspheres in Grade II and III patients were statistically significant(P<0.05).Urinary beta 2 microspheres protein in grade II and grade III patients was not statistically significant(P>0.05).5)Blood,urine beta 2 micro albumin and U-mAlb in the 1 phase of high blood pressure has begun to increase compared with control group,this has statistical significance(P<0.05),Blood and urine beta 2 micro albumin in the 2 phase and the1 phase is statistically significant(P<0.05),U-mAlb was not statistically significant(P>0.05)in the 2 phase and 1 phase.The blood,urine beta 2 micro albumin and U-mAlb in the 3 phase of hypertension and the 1 phase ratio were significantly higher,the difference was statistically significant(P<0.05).6)The positive rates of serum,urine beta2 and U-mAlb were 40.3%,79%,75.8%,the positive rate of three detections in patients with hypertension and early renal injury was 91.9%.Urine,blood beta 2 microglobulin and U-mAlb any single compared with three joint test positive rate were statistically significant(P<0.05).ConclusionCombined detection of serum,urine beta 2 micro albumin and urine micro albumin is meaningful in early diagnosis of hypertensive nephropathy. |
