Supratentorial Meningiomas Early Postoperative Epilepsy Related Factor Analysis

Objective:Though regression analysis on patient data,analyze the risk factors of early postoperative epilepsy and the factors' related degree.Aiming to evaluate the risk of early postoperative seizures.Method:Collect clinical data of 104 patients who have received craniotomy for meningiomas in our department between January 2014 and January 2016.Analyse 9relatived factors including gender,age,preoperative epilepsy,tumor location,tumor diameter,peritumoral edema,operation time,prophylactic use of antiepileptic drugs,postoperative tumor bed bleeding to observe the correlation between all factors on early postoperative epilepsy and correlation degree between the related factors.Then line logistic regression analysis of various factors on the influence of early postoperative epilepsy.Result:By analysing clinic data,12 patients occurred early postoperative epilepsy,the incidence of 11.5%.Above all data,there are 5 cases(11.9%)occurred epilepsy in men,7 cases(11.3%)in women;3 patients(17.6%)occurred epilepsy in 20 ~ 40 years,6 cases(12%)in 40 ~60 years,3 cases(8.1%)in 60 ~ 80 years;8 cases(38.1%)occurred epilepsy in patients with preoperative history of epilepsy,4 cases(5%)in patients without preoperative history of epilepsy;10 cases(16.1%)occurred epilepsy in peritumoral edema patients,2 cases(4.7%)in non peritumoral edema patients;5 cases(23.8%)occurred epilepsy in the frontal area meningioma,4 cases(22.2%)in the central area meningioma,1 case(7.7%)in temporal area meningioma,1case(6.7%)in sphenoid ridge meningioma,1 case(7.7%)in anterion cranial fossameningioma,no patient occurred postoperative seizures in the rest areas;3 patients(14.3%)occurred epilepsy in patients with tumor diameter< 3 cm,4 cases(6.8%)in patients with tumor diameter between 3 cm and 5 cm,5 cases(20.8%)in patients with tumor diameter > 5 cm;2 cases(16.7%)occurred epilepsy in patients with operation time< 2 h,6 cases(9.7%)in patients with operation time 2~4h,4 patients(13.3%)in patients with operation time >4h;7 cases(11.7%)in patients with prophylactic use of antiepileptic drugs in,5 cases(11.4%)in patients don't use antiepileptic drugs;5cases(50%)occurred epilepsy in patients with postoperative tumor bed bleeding,7 cases(7.4%)in patients without postoperative tumor bed bleeding.Single factor analysis to peritumoral edema(?2 = 12.74,P < 0.05),preoperative epilepsy history(?2 = 9.489,P <0.05),tumor location(?2 = 20.56,P < 0.05),tumor diameter(?2 = 10.653,P < 0.05),tumor cavity hemorrhage(chi-square = 8.247,P < 0.05)are associated with meningioma early postoperative seizures.Patients' gender,age,operation time,prophylactic use of antiepileptic drugs has nothing to do with seizures.Correlation from high to low are respectively for the tumor location(C = 0.406),peritumoral edema(C = 0.330),tumor diameter(C = 0.305),preoperative epilepsy history(C = 0.289),postoperative tumor bed bleeding(C = 0.271),.Logistic regression analysis showed that peritumoral edema,preoperative epilepsy history,tumor location,postoperative tumor bed bleeding,tumor diameter are risk factors for early postoperative seizures and the related risk coefficient decreases.Conclusion:The preoperative epilepsy history,tumor location,tumor diameter,peritumoral edema,postoperative tumor bed bleeding are risk factors of early postoperative epilepsy.
