Objective: the purpose of this study was to compare and analyze the similarities and differences between the two groups of patients of alcoholic liver cirrhosis(ALC)and hepatitis B liver cirrhosis(HBC)in basic data,clinical symptoms and signs,laboratory examination,complications,ultrasonic imaging features and gastroscopy.Through to explore the similarities and differences between ALC and HBC comprehensively and systematically,we could provide scientific basis for clinical workers about prevention,identification,diagnosis and treatment.Methods: This study collected 82 patients with ALC and 268 patients with HBC in First Affiliated Hospital of Dalian Medical University from January 2005 to October2015.All patients met the requirements of the experiment and had medical records completely.The 82 patients with ALC were all men,and their age was between 29 to 88 years old.In the same period,the 268 patients with HBC had 190 men and 16 women,and their age was between 28 to 82 years old.This study compared and analyzed two groups of patients in basic information,clinical symptoms and signs,laboratory examination,Child–Pugh classification of liver function,complications,imaging examination and gastroscopy.The basic information contained sex,age and course of the disease.The clinical symptoms and signs contained liver disease face,anaemic,angular,low grade fever,loss of appetite,abdominal distension,liver area pain,diarrhea,vomiting,hepatomegaly,jaundice,double leg edema,nose bleeding,liver palm,spider nevi,ascites,splenomegaly,esophageal varicose veins and abdominal wall exposed vein.The laboratory examination contained ALT,AST,AST/ALT,GGT,ALP,ALB,A/G,TBIL,DBIL,TBA,WBC,RBC,MCV,PLT,sodium,potassium,Chol,TG and UA.The complications contained HGB,HCC,HE,HRS,SBP,cholecystitis,cholelith disease and pancreatitis.The imaging examination contained portal vein diameter,liver volume,diffuse echogenicity,liver serrated change and liver capsule uneven patchy echo.The gastroscopy contained MIEV,MOEV,SEV,GU,DU and PHG.Results:1.The incidence of patients with ALC and HBC was rising trend year by year from2005 to 2015.Compared with HBC,the patients with ALC were mostly male patients,their average age was younger than HBC,and the course of disease was shorter than HBC,statistics had difference(P < 0.05).2.On the clinical symptoms and signs,the patients with ALC appeared more outstanding than HBC in liver disease face,loss of appetite,vomiting,hepatomegaly,jaundice,nose bleeding,liver palm,spider angioma and double leg edema,statistics had difference(P < 0.05).3.Compared with HBC,the indexes of ALC that reflected the degree of liver damage such as AST/ALT,GGT,TBIL,DBIL were increased significantly,statistics had difference(P < 0.05).And other laboratory related indicators such as the MCV,PT,TG,Chol and UA were increased obviously,statistics had difference(P < 0.05).4.In Child-Pugh classification of liver function,grade C of ALC was obviously higher than HBC,statistics had difference(P < 0.05).5.Compared with HBC,the patients with ALC caused HE more easily,and they had higher risk of pancreatitis,statistics had difference(P < 0.05).The HCC incidence of HBC was obviously higher than ALC,statistics had difference.6.On the imaging examination,patients with ALC appeared more outstanding than HBC in the incidence of liver volume increasing and diffuse echogenicity,statistics had difference(P < 0.05).Patients with HBC appeared more outstanding than ALC in liver volume shrinking,uneven patchy echo and liver serrated change,statistics had difference(P < 0.05).7.In gastroscopy,patients with ALC were obviously higher than HBC in mildesophageal varices,statistics had difference(P < 0.05).Patients with HBC were significantly higher than ALC in severe esophageal varices,statistics had difference(P< 0.05).Conclusion:1.The patients with ALC were mostly male patients,and the incidence was rising trend year by year.2.Liver cell injury of patients with ALC was significantly higher than HBC,and their liver reserve function was poorer.3.The patients with ALC caused HE easily,and HE was the most common cause of death.The HCC incidence of HBC was obviously higher than ALC,and the prognosis of HCC was poorer.In addition,the patients with ALC had higher risk of pancreatitis.4.The patients with ALC were mainly for liver volume increasing,while the patients with HBC were mainly for liver volume shrinking.5.The patients with ALC were prone to mild esophageal varices,and the patients with HBC were prone to severe esophageal varices.In short,the patients with ALC has its unique clinical features.
