Objective: By comparing the level of the brain natriuretic peptide(BNP)in the progressive and non-progressive,massive and non-massive,cardiac and non-cardiac of ischemic stroke in different expression levels,and exploring t he clinical value of the level of BNP on above types of acute ischemic strokeand the prognosis effect on above patients.Methods: Choosing 103 cases of acute ischemic strokediagnosed in Dandong First Hospital from May 2013 to December 2015(all were diagnosed by CT or MRI).The time from attack to arrival at hospital of all selected patients should be no more than 24 hours.The patients were divided into progressive and non-progressive according to the revised diagnostic criteria of the Fourth National Conference on cerebrovascular diseases.According to China's 2000 National Cerebrovascular Disease Symposium by typing the draft provisions for staging and treatment of cerebral infarction: the classification criteria of massive infarction is that the cerebral hemisphere is in infarction for more than 1 lobes(2 lobes or above),and the diameter of infarction is above 5 cm;and it is classified into massive infarction and non-massive infarction.It is classified as cardiac and non-cardiac cerebral infarction(large artery atherosclerosis,small artery occlusion,other etiology and unknown type)according to TOAST classification criteria of cerebral infarction.All patients were conducted the detection of the level of BNP within 1 hours and 14 days of admission to hospital.Record the scores of the NIH stroke scale,Glasgow Coma Scale and Modified Rankin Scale.Using the software package SPSS19.0 to carry on the statistics analysis.Results: The BNP level in progressive apoplexy were significantly higher than those in non-progressive apoplexy;the BNP level was significantly higher than that of non-massive ischemic strokein massive infarction;the BNP level in cardiac ischemic strokewas significantly higher than that of non-cardiac cerebral infarction;the BNP level in the patients who had higher NIHSS scores was significantly higher than that who had lower NIHSS scores before and after the treatment.;the patients who had higher BI scores had lower BNP level than those patients who had higher BI scores;the patients who had higher GCS scores had lower BNP levels than those who had lower GCS scores;the patients who had lower MRS scores after the treatment had higher BNP levels than that those patients who had lower MRI scores.Conclusion:1.The level of serum BNP were closely related to the infarct size,disease progression and severity of the disease of the acute ischemic strokepatients.2.The level of serum BNP may be related to the prognosis of patients with acute cerebral infarction.3.Serum BNP level may be used as a laboratory index for early identification of cardiac and non-cardiac cerebral infarction.
