BackgroundThe incidence of congenital microtia malformation in China is about 1:3000.Due to its congenital attribute and negative effect on patients' appearance,congenital microtia not only brings patients with physical defects,but also adversely influences their mental growth.So it is particularly important to choose the right treatment timing and surgical method.Nowadays,auricular reconstruction is the main operation method to improve the appearance defects of microtia,and it is also recognized as one of the most challenging operations in plastic surgery.At present,the most commonly used surgery methods for auricular reconstruction mainly include skin flap-extension auricular reconstruction(hereinafter flap-extension)and modified Brent or Nagata auricular reconstruction(hereinafter modified Brent).However,there is seldom research comparing the complications and postoperative effects of these two methods.Therefore,the study comparing these two surgical methods has important guiding significance to direct the selection of clinical surgery methods and to prevent postoperative complications.ObjectiveTo compare the rate of postoperative complications,effects and satisfactions between flap-extension and modified Brent auricular reconstruction,and to summarize the pros and cons as well as application conditions for both methods.Methods199 patients who have been diagnosed as congenital microtia and received auricular reconstruction from Oct 1st 2009 to Oct 31 th 2014 in our hospital were divided into two groups depending on their surgery method.Group A received flap-extension and Group B received modified Brent.Then the rate of postoperative complications,effects and satisfaction were calculated and analyzed.ResultsA total of 199 cases were included,with Group A 159 cases and Group B 40 cases.The rate of complications of Group A is obviously higher than that of Group B(p<0.05).There were no significant differences in hematoma,infection,frame exposure and skin flap necrosis between the two groups(p>0.05).Besides,there were 183 patients receiving postoperative follow-up(91.96%),with 146 cases in Group A(91.82%)and 37 cases in Group B(92.50%).The difference is not significant in postoperative effects and patients' satisfaction between the two groups(p>0.05).Conclusions1.The postoperative complication rate of modified Brent is much lower than that of flap-extension.2.There is no significant difference in postoperative effects and patients' satisfaction between the two methods.
