Objective:1.Detection the serum level of cytokines interleukin(IL)-37,caspase-1 and interleukin(IL)-18 in patients with psoriasis vulgaris in different onset period of times,to investigate the interaction of IL-37,caspase-1 and IL-18 in the pathogenesis of psoriasis vulgaris (PV)2.Detection the expression in peripheral blood mononuclear cells (PBMC) of IL-37mRNA transcription level in patients with psoriasis vulgaris in different onset period,to observe the possible role of IL-37 in the pathogenesis of PV.Methods:1.Peripheral blood was collected from patients with psoriasis vulgaris(PV)in a total of 65cases,including progressive (26 cases), stationary (24 cases), regressive (15 cases) stage and 30 healthy human controls.Divided those cases into four group, the progressive stage group, stationary group, regressive group and control group.The serum levels of caspase-1, IL-37 and IL-18 was detected by enzyme-linked immunosorbent assay.2.The expression of IL-37 mRNA in peripheral blood mononuclear cells(PBMC) was detected by reverse transcriotion-polymerase chain reaction(RT-PCR).Results:1.The serum level of IL-37, caspase-1 and IL-18 expression in patients with PV: The patients with stationary and regressive stage of PV showed a higher serum level of IL-37 compared with the healthy controls (P<0.05). While the progressive,stationary and regressive period (23.69+6.18pg/ml,94.52+35.97pg/ml, 142.67+22.89pg/ml) compared with the control group(28.80+6.60pg/ml), the difference was statistically significant (P value were< 0.05,< 0.01,< 0.01).The serum levels of caspase-1 expression in patients with PV were higher than the healthy controls (t=18.85, t=10.92, t=4.88,P<0.05).The expression levels of caspase-1 in progressive stage (134.51+24.44pg/ml) of patients was higher than that of patients with stationary period, and regressive phase (95.70+22.67 pg/ml,63.33+16.54 pg/ml, t=5.81, t=10.10),there is statistical significant differences (P<0.05).The serum level of IL-18 in patients with progressive and stationary stage of PV were higher than in healthy controls (P<0.05),and lower level with regressive stage of PV than in healthy controls (P<0.05).The serum level of IL-18 in different stage of patients with psoriasis vulgaris were 180.91+ 11.83pg/ml,124.22+11.85pg/ml,53.18+7.62pg/ml compared with control group (64.20+11.36pg/ml), the difference was statistically significant (P<0.01). 2.The expression of IL-37 in patients with PV was negatively related with IL-18(r=-0.81,P< 0.05),and the expression of caspase-1 was positively related with IL-18(r=0.89,P<0.05).3.The IL-37 mRNA expression quantity of PBMCs inprogressive stage, stationary and regressive stage were separately 1.29?2.79?5.80 times of the healthy controls.Conclusion:1.IL-37 may be involved in the pathogenesis of psoriasis, and with the interaction of caspase-1,IL-18 is involved in the development and outcome of psoriasis.2.IL-37mRNA in patients with psoriasis vulgaris quiescent period, catagen high expression,IL-37 may function as an anti-inflammatory cytokines factors in the development of psoriasis, promoting psoriasis lesions subsided.
