BackgroundThere has been an upward tendency in the cardiovascular risk factor epidemic with the vast expansion in economic and social development. Cardiovascular diseases also have been the leading cause of death in both urban and rural residents, as a result of the mounting morbidity of cardiovascular disease. The condition and evolution of Acute myocardial infarction is varied and dynamic,which largely contributes to the high cardiovascular mortality. The basic pathological change of cardiovascular diseases is atherosclerosis.Researches indicate that endothelial dysfunction involved in the whole progression of atherosclerosis.Maintaining homeostasis under the physiological condition, the vasoconstrictor endothelin and vasodilator nitric oxide derived from endothelial cells keep normal endothelial function.In some pathological conditions, the balance of the biosynthesis and release of endothelin and nitric oxide is broken and the protection of endothelial cells to cell wall,which contributes to developing AMI directly. Bosentan is a non-selective ETA / ETB receptor antagonist and is mainly used for the treatment of pulmonary arterial hypertension. However a large number of experiments have showed that it also has a crucial role in the treatment of heart failure, myocardial infarction. In this study, we measure serum concentration of endothelin and nitric oxide in patients with acute myocardial infarction and treat H9C2 cells with different concentration of endothelin and bosentan,in order to investigate the role of endothelin and nitric oxide in acute myocardial infarction. ObjectiveTo investigate the effect of endonthelin and nitric oxide on myocardial injury after acute myocardial infarction and the value of endothelin receptor antagonism as a therapeutic target. MethodsEighty-four individuals were enrolled,52 of which were diagnosed as acute myocardial infarction,and the other 32 healthy subjects were enrolled as the control group. The serum concentration of endothelin and nitric oxide were measured by ELISA kit. Treating H9C2 cells with different concentrations of endothelin or bosentan for 6 hours, then detect cell vitality and LDH. Results1.Serum endothelin level was obviously higher and NO level was significantly reduced in AMI patients than the healthy individuals(P<0.05).2.Endothelin level was positively correlated with troponin levels(r = 0.689; P = 0.000), and were positively correlated with CK-MB level(r = 0.412; P = 0.007);3.The NO level was negatively correlated with troponin levels(r =-0.51; P = 0.000), and negatively correlated to the levels of CK- MB(r=-0.366;P=0.008);4.After treated with different concentrations of endothelin for 6 hours, cell vitality was decreased and LDH level was increased obviously as the increasing concentration of endothelin.5.Cell vitality was remarkable improved and LDH decreased when treated with bosentan of different concentrations. ConclusionBosentan has obvious protective effect on myocardial injury induced by Endothelin.
