Objective: To evaluate whether the safety and efficacy of RTME in patients with rectal cancer are equivalent to those of LTME.Methodology: To retrieval the database at home and abroad by computer,the literature in the English or Chinese should be considered that was comparison of curative effect of radical operation for rectal cancer about RTME(Robot Assisted Total Mesorectal Excision)and LTME(Laparoscopic Total Mesorectal Excision)during October 2010.Methodological Index for Non-Randomized were utilized to evaluate quality of study.all meta-analyses were used Review Manager software,Version 5.3.Results: Ten studies were identified and it included 2044 patients in total.RTME930(45.5%)and LTME 1114(54.5%)in Ten studies.Meta analysis suggested that RTME group compared to LTME had longer operating time(WMD=51.19,95% CI=18.95~83.43,P=0.002),the lower rate of the conversion to open surgery(OR=0.19,95%CI=0.08~0.48,P=0.0004),slightly shorter distal margin(WMD=-0.16,95%CI=-0.31~-0.01,P=0.03)and higher International Index of Erectile Function(IIEF-5)scores after surgery(WMD=3.90,95%CI=2.27~5.53,P<0.00001),but there were no significant differences(P>0.05)in length of hospital stay,estimated blood loss,number of retrieved lymph nodes,postoperative ventilation,postoperative complications(postoperative anastomotic fistula,wound complications,ileus,urinary retention)and International Prostate Symptom(IPSS)Score after 3 months between the two groups.Conclusion: RTME is safe and feasible,and the short-term oncological and functional outcomes are equivalent to LTME.The advantages of RTME include reduced lower conversion rates and effectiveness for nerve-preserving.
