| Objective: Breast approach endoscopic thyroidectomy,which is deeply accepted by patients because of no postoperative scar cosmesis compared with conventional surgery is one of the most commonly used clinical minimally invasive surgery.However,compared with the open thyroidectomy,the safety and the wound are still controversial.The clinical treatment effect of two types of surgery will be compared and analyzed in this paper.Methods: A total of the clinical data of 62 patients after operation for treatment during January 2013 and December 2015 unilateral thyroid surgery in Department of laparoscopic surgery of the first affiliated hospital of Dalian Medical University were collected and analyzed retrospectively.The operations include unilateral benign thyroid surgery and unilateral thyroid cancer radical mastectomy.Compare the two groups of patients with length of stay,postoperative hospital stay,operative time,bleeding volume,the quantity of drainage after operation,complications and the cosmetic effects.Results: The difference of hospitalization time between the endoscopic thyroidectomy(5.41±0.95)days and open group(5.50±0.97)days was no statistically significant,statistical results is P = 0.70,P>0.05;The difference of postoperative hospital stay between the endoscopic thyroidectomy(4.41±0.95)days and open group(4.50±0.97)days was no statistically significant,statistical results is P = 0.70,P>0.05.Operative time of open group(86.33±7.18)min is obviously less than the endoscopic thyroidectomy(99.53±9.28)min.The statistical results is P=0.00,P<0.05.The difference was statistically significant.But group had the bleeding amount of the open group(25.00±9.00)ml is more than endoscopic thyroidectomy(12.50±5.39)ml.The statistical results is P=0.01,P<0.05.The difference was statistically significant.The quantity of drainage after operation of open group(56.00±13.29)ml is obviously less than the endoscopic thyroidectomy(80.00±11.64)ml.The statistical results is P<0.05.The difference was statistically significant.Drainage tube exelcymosis time: open thyroid group averaged 1 days,significantly less than the endoscopic thyroidectomy group for 3 days.The number of lymph nodes detected in the central region postoperative:OT group,4.83 ± 0.99;ET group,4.38 ± 1.07.Statistical results is P = 0.49,P>0.05;There was no significant difference.The number of parathyroid mistakenly cut:OT group,0/30;ET group,0/32.Other postoperative complications(hemorrhage,incision infection,subcutaneous effusion,facial edema): Endoscopic thyroid surgery group(2/32)more than in open thyroid surgery group(1/30).Short follow-up after the two groups were not found in tumor recurrence and metastasis.Conclusion:1.The study found that,in the surgical treatment of unilateral thyroid disease,Endoscopic thyroid surgery and open surgery in the hospitalization time and postoperative hospital stay are close,have similar hospitalization experience effect,can meet the cosmetic patients psychological expectations,and both central lymph node dissection and the rate of parathyroid mistakenly cut can reach the level.2.Laparoscopic surgery compared with open surgery has the advantages of small incision,less bleeding and satisfactory cosmetic effect.The disadvantage of laparoscopic surgery is that the operation time is longer than the open operation,serious trauma,costly.The security of endoscopic thyroid cancer surgery is not much differences compared with open surgery.3.Endoscopic thyroid surgery and open thyroid surgery are two different surgical methods of thyroid diseases.They have differences from each other,but similar effect,are satisfied with the patients.The choice of specific surgical methods should be the most likely to meet the needs of patients. |
