Objective: Breast cancer is the most common malignant tumor of female,hormone receptor status is an important indicator of prognosis of breast cancer and molecular classification.The prognosis of mammary cancer patients with hormone receptor-positive is relatively better,and the patients have endocrine therapy.Currently the clinical significance of the estrogen receptor(ER)status for evaluating the prognosis of breast cancer has been widely recognized,but with the deep research and nderstanding in progesterone receptor(PR),more and more scholars committed to explore the PR as an independent factor that influences the prognosis of patients with breast cancer.This study tried to explore whether there is a difference in clinicopathologica features and prognosis between the breast cancer patients with different PR status,and then speculate the significance of PR in breast cancer molecular classification,treatment and prognostic analysis.Methods: We restropectively analysised on the clinical data of 983 cases of breast cancer patients treated in our hospital during the period from January 1,2012 to December 31,2014,who were divided into PR + group and PR – group depending on the state of PR,then we tried to explore whether there are differences in the clinicopathologica features and prognosis between the two groups of patients.The PR+ patients were also grouped according to the different positive rate of PR,and we analyzed the clinicopathologica characteristics and prognosis of each group.Results: The ratio of invasive lobular cancer,papillary carcinoma,mucinous carcinoma in PR+ patients was higher than the PR-group.PR + breast cancer patients’ tumor histological grade was relatively lower,tumor size mainly concentrated in T1 stage and clinical stage mainly concentrated in Ia,meanwhile the tumor cells more easily expressed ER,but HER-2 gene amplification was not easy to appear and Ki-67 expression level was relatively lower.On the other hand,PR-breast neoplasm tissue histological grade was relative higher,the size of tumor mainly concentrated in T2 stage,clinical stage mainly concentrated in stage II A,and tumor cells were less likely to have ER,had more amplification of HER-2 gene and Ki-67 expression level was relatively higher.Survival analysis showed that there was no significant difference between PR+ group and PR-group in OS(P=0.574).Furthermore,we divided PR + patients into four groups of 1%-10%,11%-33%,34%-66%,67%-100% by the PR positive rate,and tried to find the differences between the four groups in the clinical and pathological features which had differences between PR+ and PR-patients.The results showed that the percentage of PR positive cells was higher;the tumor cells more easily expressed ER,were less prone to have HER-2 gene amplification and had higher expression of Ki-67.But there was no statistical differences between the four groups and the PRgroup in OS(P=0.984).Conclusions:1.In breast cancer patients,comparing with PR negative patients,the invasive lobular carcinoma,papillary carcinoma,and mucinous carcinoma had higher proportion in PR positive patients,meanwhile the tumor grade was relatively lower,the tumor size was smaller,and the clinical stage was relatively earlier.2.In breast cancer patients,comparing with PR negative patients,PR positive patients’ tumor cells more easily expressed ER,had less Her-2 gene amplification,and Ki-67 expression level was relatively lower.3.In short-term follow-up,there was no statistical difference in OS between PR positive and PR negative patients. |
