Analysis About The Relationship Between Aspirin Resistance And Platelet Update/Activation In 52 Recurrent Cerebral Infarction Cases

Objectives: To guide the clinical application of the anti-platelet drug aspirin by studying the relationship between the aspirin resistance(AR)and platelet update/activation in 52 recurrent cerebral infarction cases.Methods:52 cases met the inclusion criteria in this study.Adenosine diphosphate(ADP)and arachidonic(AA)induced platelet aggregation rate(PAg)was detected by whole blood lumi-aggregometer.ADP induced PAg ≥70% or AA induced PAg ≥20% was defined as AR,while others that did not conform to the standard were aspirin sensitivity(AS).Flow cytometry was used to detect the platelet update/activation indicators,such as reticulated platelet percentage(RP%),platelet membrane glycoprotein P-selectin(CD62P)and lysosomal protein(CD63).All cases were recorded general clinical data(including gender,a ge,history of disease,white blood cell counts,red blood cell counts,platelet counts,triglycerides(TG),total cholesterol(CHOL),high-density lipoprotein concentrations(HDL-C)and low-density lipoprotein concentrations(LDL-C)).Logistic regression analysis was used to explore the relationship between AR and platelet update/activation indicators,general clinical data,and to screen out independent risk factor of AR.The predictive value of risk factors was mearured by the area under the ROC curve.Results: In 52 cases,AR was 27(51.92%)and AS was 25(48.08%).The RP%,CD62 P and CD63 of AR cases was significantly higher than AS cases(2.98±2.72% VS 1.42±2.61%,9.89±1.27 VS 5.24±1.16,8.49±1.21 VS 4.24±1.52,P <0.01).Logistic regression analysis showed that RP%,CD62 P and CD63 were independent risk factors of AR(P <0.05),odds ratio respectively was 3.125,1.926,1.776,and 95% confidence interval(CI)was1.885-7.115,1.002-5.324,1.325-4.337.In the relative risk factor analysis,the area under the ROC curve in RP%,CD62 P and CD63 was0.765,0.607,0.601.Conclusions:More than half of recurrent cerebral infarction cases exist AR.Rapid platelet update and increased platelet activation may be one of the reasons for the occurrence of AR in recurrent cerebral infarction cases.RP% may be a good indicator that indicates AR in recurrent cerebral infarction cases.
