| Objective:Vascular dementia (VD) is a progressive disease that affects cognitive abilities and is caused by reduced blood flow to the brain.VD patients may suffer from slowed thinking, forgetfulness, depression and anxiety, disorientation, and loss of executive functions like problem solving, workingmemory, thinking, reasoning, judgment, planning and execution of tasks, with performance declining with increasing task complexity. However, the patient’s family caregivers will also because of the economic situation, long time of treatment, the patient’s cognition can cause depression, anxiety in psychological condition.Has the study found that psychological status of family caregivers in the patient’s rehabilitation process will produce important influence, should cause enough attention. In This topic, caregivers of patients with vascular dementia of suzhou Canglang hospital were adopted as research objectives, the depression and anxiety status of these caregivers were investigated and evaluated, and analyzed the related influencing factors, so as to provide basis for psychological intervention measures for family caregivers, and to improve the rehabilitation effect of patients as well as the improvement of doctor-patient relationship.Method:Selected the patients and the family caregivers from October 2014 to June 2015 in SuZhou Cang Lang hospital internal medicine outpatient and inpatient department as the research object, and score the cognitive function of the patients who got the vascular dementia included in this study.Used the Chinese version of simple intelligent scale mental state examination (MMSE) to evaluate the cognitive function of the 71 cases patients with vascular dementia involve in this research.According to the score, this study divided all the patient and family caregivers into three groups:mild, moderate and severe cognitive dysfunction, each set of sample is 39 cases,22 cases,10 cases. Scoredthe depression anxiety of the patients’ family caregivers:based on the group above, using the Hamilton depression rating scale (HDRS), Hamilton anxiety scale (HARS) to evaluate the status of family caregivers depression and anxiety.And compared the score of each family caregivers depression anxiety.For certain groups of depression and the patients with vascular dementia who are taking care by anxiety certainly family caregivers, compared the score of MMSE assessment scale of the directional force, memory, attention and computing power, memory, and language ability. Using self-made questionnaire investigated the family caregivers of general information, research content includes gender, relationship with patients, working conditions, cultural degree, take care of the time, a total of five. Compared the general information of each family caregivers, analysed the main factors influencing the anxiety depression of family caregivers. And the data processing using SPSS17.0 statistical software package.Result:MMSE score in patients with vascular dementia results show that the selected 71 cases of patients with vascular dementia MMSE score are all under 27 points,55% of the patients with vascular dementia arelight cognitive dysfunction,31% of the patients with vascular dementia are mild cognitive dysfunction,14% of the patients with vascular dementia are severe cognitive dysfunction. The family caregivers of patients with vascular dementia HDRS grading analysis according to the results:in the normal control group,4% of family caregivers are sure depression, 4% were likely to be depressed,92% no depression symptoms. Withlight cognitive dysfunction of the family caregivers of patients with vascular dementia,13%sure have depressive symptoms, 13% May have depression symptoms,74% no depression symptoms; With mild cognitive dysfunction of the family caregivers of patients with vascular dementia,23%sure have depressive symptoms,45% May have depression symptoms,32% no depression symptoms; With severe cognitive dysfunction of the family caregivers of patients with vascular dementia, 40% sure have depressive symptoms,40% May have depression symptoms,20% no depression symptoms. Compared with normal control group family caregivers, light, medium and severe group family caregivers must the incidence of depression were significantly higher (p< 0.05). Compared with patients with light cognitive impairment in family caregivers, moderate and severe cognitive impairment of patients’family caregivers must significantly higher incidence of depression (p< 0.05); And family caregivers of patients with severe cognitive impairment depression incidence group is significantly higher than moderate family caregivers (p< 0.05). The family caregivers of patients with vascular dementia HARS grading analysis according to the results:in the normal control group,12% of family caregivers are sure anxiety,15% were likely to be anxiety,73% no anxiety symptoms. With light cognitive dysfunction of the family caregivers of patients with vascular dementia,23% sure have anxiety symptoms,21% May have anxiety symptoms,56% no anxiety symptoms; With mild cognitive dysfunction of the family caregivers of patients with vascular dementia,37% sure have anxiety symptoms,36% May have anxiety symptoms,27% no anxiety symptoms; With severe cognitive dysfunction of the family caregivers of patients with vascular dementia,60% sure have anxiety symptoms,30% May have anxiety symptoms,10% no anxiety symptoms. Compared with normal control group family caregivers, light, medium and severe group family caregivers must the incidence of anxiety were significantly higher (p< 0.05). Compared with patients with light cognitive impairment in family caregivers, moderate and severe cognitive impairment of patients’ family caregivers must significantly higher incidence of depression (p< 0.05); And family caregivers of patients with severe cognitive impairment anxiety incidence group is significantly higher than moderate family caregivers (p< 0.05).Certain family caregivers of patients with depression and anxiety MMSE specific grading analysis results indicate that compared with mild group, medium and severe patients of directional power rating and language ability score was significantly lower (p < 0.01), and severe group scores significantly lower than the severe group (p< 0.01),got statistically significant. Three groups of patients with memory, attention and computing power, memory ability was no significant difference.Conclusion:This research concluded that the MMSE score of high and low will affect vascular dementia patients family caregivers of anxiety depression, The lower the MMSE score, the higherthat the incidence of depression and anxietyof family caregivers.And through the specific analysisof the sure and certain anxiety depression of family caregivers scores of MMSE indicated thateach groups of vascular dementia patient’s orientation force and language ability score had significant differences. Shows thatorientation force of cognitive dysfunction in patients with vascular dementia and language disabilities are probably lead to family caregivers main influence factors of depression and anxiety.In addition, through the reseach of family caregivers general information, including gender, relationship with patients, cultural degree, working condition and to take care of the time, analysis results show thatthe family caregivers of patients with female partners are more likely to depression anxiety, and care to more than 1 month, the incidence of depression anxiety significantly higher. |
