Study On The Clinical Manifestations And Metabolic Qizhu Granules In Treating Diabetic Nephropathy

Objective:This study is on the basis of previous animal experiments for clinical use of different treatment methods in patients with III- IV stage of diabetic nephropathy(DN), through observed the clinical symptoms and the differences between before and after the treatments on laboratory indexes and serum metabolites of Qizhu granules in the clinical treatment of early metaphase of DN curative effect, and further improve the Qizhu granules in the treatment of DN curative effect evaluation system, laying the groundwork for the Qizhu granules clinical sample test research, but also lay the foundation for clinical popularization and application.Method:(1) according to the diagnostic and inclusion criteria, from January 2015 to October 2015, 72 DN patients in a hospital renal hospital in Shanxi province were combined with traditional Chinese medicine and Western medicine, were randomly divided into 3 groups;(2) the 3 groups were combined with the specific patient needs given basic treatment and symptomatic treatment, and given the low sugar and high-quality protein diet. Diabetic nephropathy with losartan losartan group(Losartan Potassium Tablets) words plus oral 50 mg, 1 times /d; qizukeli group with Qizhu granule orally, 1 bag each time, 2 times /d;(3) a detailed record of the general data of all subjects, and the main change score certificate and a certificate, laboratory examination index data, the difference of serum metabolites and chemical shift;(4) data were analyzed using SPSS19.0 statistical software,using P<0.05 with statistical significance; using nuclear magnetic resonance spectroscopy(as H-NMR) by multivariate analysis system analysis of DN patients serum metabolites Contour,describing the change trajectory.Result:Results the clinical manifestations of study:(1) the general situation of three groups of patients before treatment, syndromes and physicochemical indexes showed no significant difference(P>0.05);(2) the clinical symptoms of qizukeli group were significantly improved after treatment, better than the other two groups(P<0.05);(3) qizukeli group DN patients with fasting blood glucose, two hour postprandial blood glucose, glycosylated hemoglobin value decreased significantly, better than the other two groups(P<0.05);(4) microalbuminuria qizukeli group DN patients, 24 hours urine protein,serum creatinine, blood urea nitrogen and serum cystatin levels significantly decreased, obviously better than the foundation treatment group(P<0.05), but the qizukeli group and the Losartan group,the two groups were not significantly different(P>0.05);(5) blood lipid level qizukeli group DN patients improved significantly better than the other two groups(P<0.05);(6) qizukeli and foundation treatment group, losartan group of liver function blood routine and blood pressure. No significant difference(P>0.05).Metabolomics research results:(1) DN patients were detected by the major metabolite peak 32 species, and other scholars' research results are similar;(2) DN patients and human health compared with 21 different metabolites, including sugars, lipids, amino acids and other metabolites or intermediate product, but with three types of metabolites or intermediate products mostly;(3) DN patients after three groups of different treatment, serum metabolites appear different degrees of callback, with the most kinds of Qizhu granules group callback.Conclusion:Qizhu granules can be significantly improved in patients with DN clinical symptoms, improve patient long-term lipid disorder and decreased significantly level in patients with urinary protein,protect renal function, delay the progress of the disease, in patients with diabetic nephropathy early mid treatment is safe and effective, the treatment is not sub Yukesuya. Also, effects of Qizhu granules on serum metabolites in patients with DN callback including glucose metabolism in the body, the metabolites and intermediate products produced in the process of lipid metabolism and amino acid metabolism, objectively reflect the Qizhu granules can effectively improve the DN patients the levels of blood glucose and blood lipid, protect renal function in patients with, clinical curative effect is exact.
